Different positioning: the lawyer on duty provides legal help for suspects and defendants who make money from crimes free of charge. The lawyer on duty does not provide "defense", but only "legal help". Therefore, the lawyer on duty has no status as a defender, only as a legal helper. Ordinary lawyers provide legal services free of charge and have the status of defenders.
