Patents are handled by an agency. Can the patentee declare a change by himself if the first patentee is changed?

If a patent agency is not entrusted, the formalities for changing the bibliographic items shall be carried out by the applicant (or patentee) or Its representative shall handle the matter; if a patent agency has been entrusted, the patent agency shall handle the matter. Changes caused by the transfer of rights can also be handled by the new right holder or the patent agency entrusted by him.

The above quote is from the Patent Examination Guidelines 2010. Is it clear?

If the first patentee is changed and the original patentee proposes the change, he must go to the original agency to handle it; if it is a new patentee, there is no need to go to the original agency
