What are the causes of cervical hyperplasia?

What are the clinical manifestations of cervical hyperosteogeny?

Cervical hyperostosis and cervical spondylosis are not the same thing. Cervical hyperosteogeny is a state, and cervical spondylosis is a disease. In the case of long-term chronic injury of cervical vertebra, cervical intervertebral disc disease, ligament injury, facet joint disorder, hyperosteogeny, etc. It may happen and lead to a series of clinical manifestations before it can be called cervical spondylosis. According to the degree and location of cervical neurovascular compression by hyperplastic cervical vertebrae, it can be divided into five types:

(1) nerve root type: the cervical nerve root is compressed by hyperplastic bone, resulting in corresponding manifestations. The incidence of cervical spondylosis accounts for 60%. Diagnostic points: ① Most of them are between 40 and 60 years old, with more males than females. ② It has typical root symptoms, such as neck, shoulder and arm pain, numbness and wrist and finger radiation pain, and its range is consistent with the area innervated by cervical spinal nerve. ③ The kinetic energy of neck movement is impaired, especially the function of extension and rotation is limited. ④ Tenderness of spinous process, compression test of intervertebral foramen and traction test of brachial plexus were positive. ⑤ X-ray film of cervical vertebra showed hyperosteogeny of posterior edge of vertebral body and uncinate process joint.

(2) Vertebral artery type: Vertebral-basilar artery spasm and insufficient blood supply caused by hyperplastic bone compression or stimulation of vertebral artery. The incidence of cervical spondylosis is about 10% ~ 15%. Diagnostic points: ① Patients over middle age often have dizziness, migraine, visual impairment, dysphonia, tinnitus, deafness, cataplexy and other symptoms due to head and neck changes. ② At the onset, the patient's neck movement is often limited, and the neck rotation induction test is positive. ③ Anterior and oblique X-ray films of cervical vertebra showed hyperosteogeny of uncinate joint and displacement of diseased vertebral body.

(3) Spinal cord type: It is mostly caused by hyperplastic bone and degeneration of cervical intervertebral disc protruding into spinal canal and compressing spinal cord, accounting for about 65,438+00% ~ 50%. Diagnostic points: ① Patients over middle age have numbness, soreness and burning pain in their limbs, and feel "stepping on cotton" when walking, with unstable body center of gravity and easy to fall. ② Muscle tension of limbs is increased, muscle strength is weakened, tendon reflex is hyperactive, knee reflex is weakened, and pathological reflex appears. ③ Comparative examination of cervical X-ray films showed hyperosteogeny, stenosis and obstruction of cervical spinal canal at the posterior edge of vertebral body.

(4) Esophageal type: It is caused by hyperosteogeny in the front of cervical vertebra pressing the posterior wall of esophagus. This type is rare. Diagnostic points: ① Age above middle age. ② The patient has throat discomfort, foreign body sensation and dysphagia. ③X-ray film showed hyperosteogeny at the anterior edge of vertebral body.

(5) Sympathetic nerve type: hyperosteogeny compresses the sympathetic nerves on both sides of the cervical spine. This type is also rare. Diagnostic points: ① Age above middle age. (2) one side of the eye fissure is big and the other side is small, the pupils are unequal, and sometimes things are not clear. Half of the face is dry, sweating less, etc. ③X-ray film showed hyperosteogeny of cervical vertebra.

How to prevent and treat cervical hyperosteogeny?

(1) Prevention: Because hyperosteogeny is related to age, sex, race, occupation, type of work, physique, poor posture and other factors. The so-called prevention refers to the measures taken by human beings against the factors that can be intervened. For example, try to avoid bowing, bowing and tilting for a long time. After work, you should do proper neck exercise, slowly, not too much. Massage yourself on your neck. Physical exercise and weight control.

(2) Treatment: Physical therapy and cervical traction include cervical support and traction frame. Drug treatment: while cooperating with physical therapy and traction, take some non-steroidal anti-inflammatory drugs appropriately.

(3) Introduce a kind of health-care neck pillow: Through the introduction of diseases such as hyperosteogeny and osteoarthritis, let patients have a certain understanding of the diseases. We can't change factors such as aging, heredity and gender, but we can focus on preventing acquired factors such as occupation, physique and bad posture. About13 of our life is spent in bed, and the neck is closely accompanied by the pillow, but how many people have noticed the influence of the pillow on the cervical spine? At present, many pillows do not meet the physiological requirements of the cervical spine, which makes the cervical spine in a state of suspension and abnormal stress for a long time, leading to chronic injury of the cervical spine. After years of research and development, someone has obtained the national patent of health-care neck pillow (patent number 942 174-437). Its characteristics are as follows: ① Due to the arc surface structure consistent with the curved surface of the neck, the traditional pillow has been changed, and the neck has been damaged by external forces that do not conform to the biomechanical characteristics for a long time. ② The pillow core with definite curative effect has the functions of nourishing the heart and strengthening the brain, inducing resuscitation with fragrance, promoting blood circulation and calming the nerves. When people sleep, they play a role in preventing and treating diseases to a certain extent. Of course, its preventive function exceeds its therapeutic function.

What is the mechanism of traction therapy for cervical hyperosteogeny?

Cervical traction is one of the effective measures to treat cervical hyperosteogeny, and it is a commonly used clinical treatment method. Its action mechanism is:

(1) Restricting neck movement is beneficial to the regression of congestion and edema.

(2) relieve neck muscle spasm and relieve pain.

(3) Enlarge intervertebral space and intervertebral foramen, and relieve the compression of nerve root, vertebral artery and spinal cord caused by bone spur.

(4) Open the synovium of impacted facet joints.

(5) Restore the normal position of cervical intervertebral joint. Under pathological conditions, the affected joints may have posture changes such as rotation, distortion and trapezoid. Through traction, the head and neck can be in a normal physiological curve state, and the curve of cervical vertebra can be restored to normal.

(6) Reduce the local inflammatory reaction of bone spur. Edema, congestion, exudation, etc. occur due to the stimulation and oppression of hyperplastic bone spurs on surrounding tissues. Can be produced locally, causing pain symptoms. The fixation and braking caused by traction can make it quickly fade away, thus relieving symptoms.

What traction therapies are commonly used to treat cervical hyperosteogeny?

(1) Sitting traction: The patient sits on a stool, fixes the mandible and occipital bone with four headbands, and draws vertically upwards. The body weight can reach 10 ~ 20kg, 1 ~ 2 hours each time, 1 ~ 2 times a day. The traction time and weight can be increased or decreased according to the patient's reaction.

(2) Recumbent traction: The patient lies on his back on the bed, and the foot of the bed on his head is raised. He uses four headbands to draw at an angle of 300 with the longitudinal axis of his body, weighing 3kg, and rests every 2 hours 1 hour, which can be repeated 1 day. 1 month is the course of treatment.

Most patients with cervical spondylotic radiculopathy can be cured by traction. In order to limit the movement of the neck, you can wear a collar.
