On March 3, 2008 16: 30, a suffocation accident occurred in the bar factory of Baosteel Special Steel Branch, causing two deaths and slightly inhaling nitrogen.
After the accident, Vice Mayor Ai Baojun and Deputy Secretary-General Xiao of the Municipal Government successively gave instructions on accident rescue, and the leaders of the Municipal Safety Supervision Bureau made specific arrangements for the rescue and disposal work. According to the Regulations on Investigation Report and Handling of Production Safety Accidents (Order No.493 of the State Council) and the Notice of Shanghai Municipal People's Government on Implementing the Regulations on Investigation Report and Handling of Production Safety Accidents (No.77 of Hu Fu Fa [2007]), the accident investigation team composed of the Municipal Federation of Trade Unions, Baoshan Public Security Bureau, Baoshan District Supervision Committee and Baoshan District Procuratorate rushed to the scene to investigate and collect evidence. The relevant information is hereby notified as follows.
After the accident, Shanghai Safety Supervision Bureau, Shanghai Federation of Trade Unions, Shanghai CDC, Baoshan District Public Security Bureau, He, deputy general manager of Baosteel Group Co., Ltd., and Fu Zhongzhe, general manager of the joint-stock company, immediately rushed to the scene to understand and inspect the scene of the accident, and went to the hospital to care for and arrange rescue work. Zhu and Guo Kezhong, the safety leaders of group companies and joint-stock companies, went out on business trips, called the site to understand the accident situation, and made specific arrangements and requirements for the on-site accident analysis and after-treatment work.
I. Brief introduction of the accident
On March 3, 2008 16: 30, the No.5 door of MCF continuous annealing furnace in pickling and heat treatment branch of Baosteel Special Steel Company broke down. When team leader Jiang (male, 26 years old, from Shanghai) and deputy team leader Wu Lingchao (male, 26 years old, from Shanghai) entered the furnace from Gate 6 without turning off the nitrogen switch, they suffocated and fainted. About 30 minutes later, other personnel found out that Dongchun Li (male, 27 years old, from Shanghai), Xu Qiang (male, 27 years old, from Shanghai) and Ma Xianhong (male, 33 years old, from Suqian, Jiangsu), the shift leader, went into the furnace to rescue them, and all three fainted. The rest of the staff immediately rescued Xu Qiang and Dongchun Li. When Dongchun Li woke up, he reminded rescuers to turn off the nitrogen switch. After the rest of the people filled the air, they rescued Ma Xianhong, Jiang and Wu Lingchao successively. These five people were rushed to the Third People's Hospital affiliated to Shanghai Jiaotong University School of Medicine for emergency treatment. Jiang and Wu Lingchao died after being rescued at 20: 00 and 22: 50 respectively, and the other three were separated from hyperbaric oxygen at 20: 15.
Second, the preliminary cause analysis of the accident
After preliminary analysis, the main cause of the accident was that the operator did not turn off the nitrogen switch and conduct effective ventilation, and at the same time did not take any effective protective measures, and went into the furnace to check the fault. The failure of rescuers to effectively identify the risk factors is an important reason for the further expansion of the accident.
Third, the next preventive measures
1. Because this accident has many similarities with last year's "3.2 1" accident, there is blindness in equipment operation and rescue work. The safety supervision department immediately informed the whole company of the accident and asked all relevant units to learn from the accident and draw inferences from others to prevent similar accidents from happening again.
2. On March 4th, Special Steel Branch organized the leaders of secondary departments to hold an accident site meeting at the strip steel plant to inform the accident process, take effective preventive measures according to the reasons of preliminary analysis, find out the deficiencies in management, formulate effective measures technically, draw inferences from others, find hidden dangers around them and take corrective measures immediately.
3. On March 4th, the company will convene relevant safety experts to form a working group to station in Special Steel Branch to find out the cause of the accident and analyze the weak links in management, so as to effectively implement the "four misses" of the accident.
4. On March 5th, the Group Company will organize all subordinate units in Shanghai to hold an accident site meeting in Special Steel Branch. In view of the accident, all units are required to conduct a comparative inspection to ensure production safety and personnel stability during the "two sessions".
