In order to successfully carry out the review of high-tech enterprises in our province in 20 14, according to the provisions of the Administrative Measures for the Accreditation of High-tech Enterprises (No.[2008] 172 of the State Science and Technology Commission, hereinafter referred to as the Accreditation Measures) and the Administrative Guidelines for the Accreditation of High-tech Enterprises (No.[2008]362 of the State Science and Technology Commission), combined with the provisions of our province's high-tech enterprises,
First, report the timetable.
This year, applications for reexamination of high-tech enterprises will be accepted in two batches. All enterprises should select appropriate application batches and submit application materials for review within the validity period of the quality qualification certificate. If they miss the time, they will not be able to fill in the application materials.
(a) the first batch of declaration schedule
The deadline for enterprises to submit online application materials on the "Sunshine Government Platform for Science and Technology Business Management in Guangdong Province" (website:/,hereinafter referred to as "Government Platform") is June 2065438+June 65438+July 2004; The deadline for accepting paper application materials in the window is July 4, 20 14.
(two) the second batch of declaration schedule
The deadline for enterprises to submit online application materials on the "government platform" is 2065438+July 25, 2004; The deadline for accepting paper application materials at the window is 2014 August 15.
Second, the reporting conditions
(1) For the high-tech enterprise recognized in 20 1 1 year, the certificate is within the validity period and meets the relevant provisions of Article 10 of the Accreditation Measures. Enterprises recognized in 2008, 2009 and 20 10 but failed to pass the re-inspection can no longer apply for re-inspection this year, and must re-apply for the recognition of high-tech enterprises according to the accreditation procedures.
(2) If the name of the reexamined enterprise changes, fill in the application for changing the name of the high-tech enterprise on the "High-tech Enterprise Identification Management Network" (website:/,hereinafter referred to as "National High-tech Enterprise Network"), and submit the paper application materials for renaming at the same time, and apply for reexamination after renaming.
Third, the declaration procedure.
(1) Online Registration
Enterprises register through the "government platform" to obtain user names and passwords. When registering an enterprise, you need to fill in the basic information such as company name, organization code, company manager information, competent department and company contact information. By default, new registered units use the organization code as the login account of "government platform", and must fill in the organization code accurately when registering enterprises. Each enterprise can only register one account on the "government platform". Enterprises that have registered in the former Guangdong Science and Technology Business Integrated Management System do not need to re-register, and directly use the original user name and password to log in to the "government platform". Please refer to the attachment 1 for online registration instructions.
(two) fill in and submit the application materials for review.
1. Fill in and improve the basic information of the company. The enterprise logs into the "government platform" with the account of company administrator, and fills in the basic company information and company financing information (if any) under the function menu of "System Management-Company Information Management".
2. Fill in the application form. In the module of "Application Management-Project Application-Fill in the Application for Evaluation of High-tech Enterprises", fill in the application information for evaluation as required, upload the attachment materials one by one, and complete the online declaration.
3. Download and print the paper application form. After the application is completed, click the "Download PDF" button to download and print the PDF document of the reexamination application with the watermark number, and bind it with the relevant application certification materials and attachments, and submit it to the local science and technology management department.

Four. Requirements of application materials
(a) the contents of the application materials
The application materials are divided into online application materials and paper application materials.
1, online application materials
Enterprises fill in relevant application information and upload attachment materials according to the requirements of the "government affairs platform" module. Online application materials must be consistent with paper application materials.
2. Paper application materials
The applicant shall, according to the actual situation of the enterprise, bind the paper application materials in the following order:
(1) Watermark applications audited by high-tech enterprises are generated after filling in "government platform" (download PDF).
(2) Copy of business license, organization code certificate and tax registration certificate.
(3) Screenshots of the inquiries made by the local tax system about the handling of the detailed declaration documents of enterprise individual income tax in June 2065 438 and June 2003 and June 2065438+February 2003 (just summarize the figures and affix the official seal of the enterprise).
(4) Copies of 20 1 1 to 20 13 annual enterprise income tax returns (main table and table 1) stamped by the competent tax authorities.
(five) the financial audit report of the enterprise in the last three fiscal years (including balance sheet, profit and profit distribution statement, cash flow statement and notes, if the actual operating period is less than three years, the actual operating period should be provided).

(7) Materials for R&D activities: project reports, project initiation and project approval certificates of science and technology plans at or above the provincial level; Interim report; The closing report involves test analysis report, project acceptance materials, published papers, test products and samples, product quality inspection report, etc.
(8) Intellectual property materials: the intellectual property certificate (authorization), the abstract published on the official website of the State Intellectual Property, the change certificate issued by the relevant competent department, the patent exclusive license contract (agreement) with a global exclusive license for more than 5 years, the filing and approval certification materials, etc.
(9) Proof materials for the transformation of scientific and technological achievements: the source of achievements can provide proof materials such as patents, technical know-how and project approval certificates. The transformation results can provide materials from the aspects of production approval, proof of popularization and application of new products or technologies, product quality inspection report, etc.
(10)R&D organization and management materials: R&D project establishment management, R&D investment accounting system, R&D organization construction and equipment and facilities, Industry-University-Research cooperation activities, R&D personnel appraisal system, etc.
(7)~( 10) supplementary materials shall be provided by the enterprise according to the actual situation.
(2) Requirements for the preparation of application materials
1. After filling in the form, the "Government Platform" generates an application for evaluation of high-tech enterprises with watermark. The application is bound with relevant attachment materials, and all kinds of attachment materials are classified at the back of the application.
2. All paper application materials shall be bound into volumes in the prescribed order, and the total page number shall be compiled page by page, and the general catalogue of materials and the corresponding page number range shall be provided in each application material.
3. Paper application materials shall be bound with glue in triplicate, of which 1 original and 2 copies.
Verb (abbreviation for verb) job requirements
(a) the municipal science and technology department is responsible for the summary and recommendation of enterprise application materials. The science and technology departments of all localities and cities should strictly examine the contents of the application materials for enterprise evaluation, and resolutely prevent and put an end to fraud. After receiving the application materials from enterprises, the science and technology departments of cities (cities in the Pearl River Delta extend to county-level science and technology departments) organize relevant departments to conduct on-the-spot verification of the application information of enterprises (high-tech zone enterprises, please ask the science and technology departments of high-tech zones to assist the science and technology departments of cities or counties to do a good job in enterprise verification), objectively record the problems found in the verification, and the science and technology departments of cities or counties shall fill in the preliminary verification opinion form (Annex 3) online, together with the paper application materials of enterprises. After being summarized by the municipal science and technology departments, it will be sent to the provincial science and technology department, and at the same time fill in the declaration summary table (Annex 4), and send the electronic version to the working contact personnel of the high-tech development and industrialization department of the provincial science and technology department through the "provincial high-tech identification working group".
(two) the science and technology departments at all levels should actively contact the local finance and taxation departments, strengthen the organization and coordination of the evaluation work, organize the publicity work of the evaluation of high-tech enterprises, and strengthen the guidance and service for the evaluation of enterprises.
(three) the reporting enterprise should not trust the promises of various social intermediary agencies, but should be responsible for the authenticity of the application materials. If there is fraud, once found and verified, the office of the leading group for the identification and management of high-tech enterprises in the province will cancel its qualification in accordance with the relevant provisions of the identification methods, and will no longer accept the application for identification of the enterprise in the next five years.
