The full implementation of the teacher qualification system is of great significance for administering education according to law, safeguarding teachers' legitimate rights and interests, strengthening standardized management of teachers, and promoting the healthy, coordinated and harmonious development of education in provincial capitals. Combined with the specific situation of Fuzhou, the relevant matters concerning the collation of teacher qualification data of normal education graduates from 2002 to 2007 and the identification of in-service teachers and teachers' qualifications are hereby notified as follows:
First, the scope of the teacher qualification object
With the "Teacher Law" and "Teacher Qualification Regulations", the following objects can apply for the qualification of teachers in primary and secondary schools (including secondary vocational schools).
1, a teacher-training graduate whose household registration or work unit is in Fuzhou from 2002 to 2007;
2, has obtained the teacher qualification, now due to the adjustment and merger of schools or work needs should obtain a new corresponding type of teacher qualification;
3. Secondary vocational schools and private schools employ on-the-job personnel who have not obtained teacher qualifications.
In any of the following circumstances, the procedures for the qualification of teachers shall be suspended:
1, failed the exam in 2006-2007;
2. Unable to continue to engage in education and teaching due to physical reasons;
3, has obtained the teacher qualification, apply for two or more teachers' qualifications;
4. After graduating from normal education, he left the education and teaching post for five years.
Two, the types of teachers' qualifications and the qualifications they should have.
1, primary school, kindergarten teacher qualifications, should have graduated from secondary normal school or secondary kindergarten normal school and college degree or above, other secondary school graduates, can not participate in primary school, kindergarten teacher qualification.
2, junior high school, junior vocational school culture, professional teacher qualifications, should have a higher normal college or other college graduates and above.
3, senior middle school teachers, secondary specialized schools, adult secondary schools, technical schools, vocational high school culture courses, professional courses teacher qualifications, should have a normal university bachelor degree or above.
4. Students in secondary specialized schools, technical schools and vocational high schools should obtain the qualification of internship instructors, have graduated from secondary vocational schools or above, and have professional and technical positions equivalent to assistant engineers or intermediate workers or above.
The teacher qualification application can only declare the teacher qualification type corresponding to the above education. For those who do have special skills, they can apply for the qualification certificate of internship guidance teachers in secondary specialized schools, and their academic qualifications can be appropriately relaxed, but they can only apply after the school (unit) reports to the Personnel Department of the Provincial Department of Education for approval.
In principle, middle school teachers and middle school teachers should fill in the teaching subjects consistent with their majors.
Three, apply for teacher qualification need to provide the following materials
1. Four two-inch bareheaded recent photos of the applicant (both black and white and color). Used for 2 photos of Teacher Qualification Application Form (self-attached), applicant 1 Physical Examination Form (self-attached) and 1 Teacher Qualification Certificate, with the applicant's name and unit written on the back of the photos and packed in sealed bags.
2. The Application Form for Teacher Qualification is in duplicate (copying the application form is not allowed). The relevant schools (units) should identify the on-the-job personnel who do not have the professional and technical positions of teachers according to the basic quality and ability conditions of education and teaching stipulated in Annex 2 of the Notice on the Qualification Identification of Primary and Secondary School Teachers in 2006, and indicate the identification conclusion of the applicant's basic quality and ability of education and teaching in the Teacher Qualification Application Form. Those who fail to pass the conclusion on the basic quality and ability of education and teaching will not be declared.
3. A copy of the applicant's physical examination form. The relevant county (city) District Education Bureau shall organize physical examination in hospitals at or above the county level; Those who apply to Fuzhou Education Bureau for teacher qualification will have a physical examination at the Health Examination Center of a hospital in Fuzhou (address: Dadao Road, Taijiang District, Fuzhou 190), and the specific time is to be determined.
4, in-service teacher professional and technical positions (titles) certificate copy and school (unit) comprehensive evaluation of qualified "2006-2007 school year evaluation registration form" copy.
5, "the applicant's ideological and moral appraisal form"
(1) On-the-job personnel shall be filled in by the school;
(2) Those who have not obtained the teacher qualification certificate, and whose file relationship is stored in the education talent market of our city and county (city), and those who have registered in Fuzhou from 2002 to 2007 (including those who were recruited in Fuzhou in 2007), shall be filled in by the township (street) where the household registration is located or the police station, or by the file storage unit according to the relevant identification materials of the institutions where the graduates attend.

7. Copy of ID card.
8. Copy of Grade II or above Putonghua certificate.
9. Achievements in pedagogy and psychology. Normal graduates and those who continue their studies to improve their academic level after graduation shall submit the original and photocopy of the Academic Achievement Form of Normal Graduates. Applicants who have graduated from non-normal education must provide the examination certificates of "Pedagogy" and "Psychology" issued and printed by the provincial self-study examination office before August 2002, or the copies of the graduation certificates of "Pedagogy" and "Psychology" courses organized by Fuzhou Education Bureau.
10. According to the documentsNo. [2002] 150 andNo. [2004] 12, private schools can apply to be recognized as in-service teachers. The school should provide the salary roster (photocopy), on-the-job certificate (photocopy) and teaching certificate, and provide the proof that the labor and social security department pays the old-age insurance premium.
The above materials shall be audited by the applicant school (unit), and the graduates majoring in normal education shall be put into the applicant's "Teacher Qualification Material Bag" after being audited by the accreditation body. The copy must be verified by the school (unit) or appraisal institution according to law, and the signature of the agent and the seal of the school (unit) or appraisal institution are valid. After approval, the original should be returned in time.
School (unit) and the applicant, the relevant materials are missing, the accreditation body will not notify the supplementary report when it is recognized, and the applicant and the school (unit) will be responsible for it.
Those who have obtained the teacher qualification, the Application Form for Teacher Qualification 1 copy shall be stored in my personnel file, and the rest materials shall be filed and kept by the teacher qualification accreditation institution. County (city) district teacher qualification accreditation institutions to establish a teacher qualification management database.
Four, the application and identification of teacher qualification
1, kindergarten, primary school, junior high school (including private) teacher qualifications, the county (city) District Education Bureau is responsible for the identification. Senior middle school teachers (including private) and teachers' training school teachers' qualifications are carefully examined by the accreditation body of the county (city) District Education Bureau and reported to Fuzhou Education Bureau for accreditation.
2. Secondary schools, primary schools and kindergartens directly under the Fuzhou Municipal Education Bureau, secondary schools and secondary vocational schools (including private secondary schools and secondary vocational schools) within the jurisdiction of Fuzhou City, and Fuzhou normal graduates who did not obtain teacher qualification certificates from 2002 to 2007, their teacher qualifications were recognized by the Fuzhou Municipal Education Bureau.
3. In line with the principle of territorial management, the applicant for teacher qualification should apply to the county (city) District Education Bureau or school (unit) with graduation certificate and ID card on September 12- 14, and submit the basic materials stipulated in Article 3 of this article. The registration place of normal education graduates recognized by Fuzhou Education Bureau is in Fuzhou Education Talent Service Center (address: Room 6 17, Fuzhou Education Hotel next to Qunzhong Road Primary School, contact: Teacher Ke, tel: 83340647, the same below).
The relevant county (city) District Education Bureau and schools (units) will submit the applicant's materials and the teacher qualification roster recognized by Fuzhou Education Bureau in duplicate on September 24-28, with electronic documents attached, and send them to Fuzhou Education Talent Service Center for review. If you fail, don't accept it.
County (city) District Education Bureau should print the teacher qualification application form, the applicant's ideological and moral appraisal form, the applicant's physical examination form and the teacher qualification applicant roster according to the format stipulated by the Ministry of Education and the Provincial Department of Education. The relevant schools (units) should go to the county (city) District Education Bureau to get it. The object identified by Fuzhou Education Bureau, the school (unit) where the in-service teacher is located, brought a letter of introduction to Fuzhou Education Talent Service Center on September 5-7, and the individual did not accept it. Normal graduates can get relevant application forms by registering at Fuzhou Education Talent Service Center within the specified time.
4, the county (city) District Education Bureau teacher qualification accreditation body shall make a conclusion on whether or not to recognize within 30 legal working days from the date of termination of the application period, and inform the applicant of the recognition result, which will be conveyed to the applicant by the school (unit).
Five, the teacher qualification certificate
Teacher qualification certificate is the legal certificate of teacher qualification, which is uniformly printed by the Ministry of Education and held by the teacher himself. Fill in the "Teacher Qualification Certificate" with neat handwriting and standard writing, and fill it in carbon ink uniformly.
"Teacher Qualification Certificate" and "Teacher Qualification Application Form" shall be numbered uniformly by the teacher qualification accreditation institution in accordance with the provisions of the state, and shall take effect after being stamped with the official seal and steel seal of the corresponding government education administrative department.
Teacher qualification certificate number. The number of the certificate and the application form is the same, both of which are *** 17 digits. The format and meaning are as follows:

Annual code: refers to the annual number of teacher qualification. For example, in 2007, the annual code is "2007".
Province code: refers to the number of the unit determined by the province, autonomous region or municipality directly under the Central Government where the teacher is located. The code of Fujian Province is "35".
Accreditation authority code: refers to the number of units (including provincial, district, city and county education administrative departments) that have the right to accredit teachers' qualifications in this province.
The codes of authorized institutions in Fuzhou are "10 1", Gulou 102, Taijiang 103, Cangshan 104, Jinan 105 and Mawei1005. Fuqing 1 1 1, Pingtan 1 12, Lianjiang 1 13, Luoyuan1/kloc.
Type code: refers to the type number of teacher qualification.
"1" stands for kindergarten teacher qualification.
"2" stands for primary school teacher qualification.
"3" stands for junior high school teacher qualification.
"4" stands for high school teacher qualification.
"5" stands for the qualification of teachers in secondary vocational schools.
"6" stands for the qualification of practice instructor in secondary vocational schools.
"7" stands for the qualification of college teachers.
Gender code: refers to the gender code of the holder.
"0" stands for "male" and "1" stands for "female"
Serial number: refers to the serial number of the teacher qualification certificate recognized by the accreditation authority. The numbering method is as follows: All teacher qualification certificates issued by the teacher qualification accreditation authority will be numbered in chronological order, and should be renumbered after changing the year.
Six, the teacher qualification fee
According to document No.34 [2006] of the Ministry of Finance and the National Development and Reform Commission, the education administrative department at or above the county level shall allocate funds for the identification, printing and distribution of teachers' qualification certificates through departmental budgets.
Citizens who have passed the teacher qualification examination, citizens who meet the teacher qualification requirements stipulated in the Education Law, as well as in-service teachers and retired teachers, shall not charge any fees for applying for teacher qualification. For those who have passed the teacher qualification certification, the "Teacher Qualification Certificate" will be issued free of charge.
"Teacher Qualification Certificate" issued by the Ministry of Education, the county (city) District Education Bureau teacher qualification accreditation agencies can receive it from the Personnel Department of Fuzhou Education Bureau as required.
Seven, conscientiously do a good job in the collation and submission of teacher qualification information.
In order to improve the management information system of teacher qualification certification and improve the informatization level of teacher qualification certification, according to the requirements of "Letter on Doing a Good Job in Collating and Submitting the Data of Teacher Qualification Certification" issued by the Teacher Department of the Ministry of Education and Document No.5 issued by the Fujian Education Office of the Provincial Department of Education [2007], the county (city) district education bureaus and schools are requested to do a good job in collating and submitting the data of teacher qualification certification.
1, data information arrangement
County (city) District Education Bureau teacher qualification accreditation agencies should seriously organize and do a good job in sorting out and summarizing the data of teacher qualification accreditation in counties (cities) from the implementation of the teacher qualification system (including the transition period and the pilot accreditation time) to 2006, and establish and improve the teacher qualification management database.
2. Content and time of submission
(1) The Education Bureaus of counties (cities) and districts will bind the teacher certification materials from the implementation of the teacher qualification system (including the transitional period and the pilot certification time) to 2006 (the statistical table of teacher certification from the transitional period to 2006 is attached) and the copy of the teacher certification applicant list in 6544.
(2) All secondary specialized schools, primary and secondary schools (kindergartens) directly under the central government, private schools (units) and other educational institutions should carefully register the in-service teacher qualification certificate, take the school as the unit, fill out the Fujian Province Teacher Qualification Confirmation Form, attach an electronic document, and bind a copy of each teacher's teacher qualification certificate into a book (the copy format is A4 paper). Schools affiliated to the county (city) District Education Bureau (including private schools) are in triplicate, one for the county (city) District Education Bureau and one for the school, and the other is summarized by the local county (city) District Education Bureau and submitted to the Personnel Department of the Municipal Education Bureau; All kinds of secondary specialized schools at all levels, primary and secondary schools directly under the municipal government, private schools directly under the municipal government (units) and other educational institutions are in duplicate, one kept by the school and the other by the personnel department of the Municipal Education Bureau. County (city) District Education Bureau and related schools should submit the materials to the Personnel Department of Fuzhou Education Bureau for the record before June 20th 10.
Education administrative departments at all levels and schools (units) at all levels should attach great importance to it, strengthen leadership, and designate special persons to be responsible for ensuring the successful completion of the work of teacher qualification identification and the data collation and submission of teacher qualification identification.
