In order to improve the efficiency of processing applications, the applicant should describe the required information as clearly as possible; If possible, please provide the title, release time, document number or other tips to help the government and health administrative departments determine the information carrier.
2. Submit the application form.
The applicant can fill in the complete application form through the website of the government and the health administrative department, by letter or in person. If you apply by letter, please indicate the words "government information disclosure application" in the lower left corner of the envelope; Applicants can also apply to the accepting institution on site.
The accepting institution does not directly accept the application for government information disclosure made by telephone, but the applicant can consult the application procedure by telephone.
Application processing: after receiving the application, the accepting institution will conduct a formal review on whether the elements of the application are complete, and return the application with incomplete elements and ask the applicant to make corrections. If the information applied for belongs to the information that has been voluntarily disclosed by the administrative organ, the accepting institution shall suspend the application acceptance procedure and inform the applicant of the ways and means to obtain the information.
Time limit for application processing: The accepting institution shall reply within 15 working days after accepting the matters disclosed according to the application according to the workflow.
