2. Accepted:
After receiving the relevant formalities submitted by the applicant, the county-level Disabled Persons' Federation will check the applicant, photo, ID card and household registration book, and input the relevant information in the application form into the basic database of the disabled population. Those who fill in false information will not be admitted.
3. Disability assessment:
Applicants who apply for a disabled card for the first time and applicants who apply for a first-generation disabled card for the second-generation disabled card, if the county-level disabled persons' Federation has obvious disability characteristics and is convenient to identify the disability category and grade according to the disability standard, can directly fill in the evaluation form, and clearly record the disability characteristics and intuitive evaluation in the evaluation form, but it must be jointly evaluated and signed by three people, including the chairman; Other difficult to directly identify the category and level of disability must be assessed by hospitals or specialized medical institutions at or above the county level designated by the county-level disabled persons' Federation, and the hospitals or specialized medical institutions at or above the county level designated by the county-level disabled persons' Federation should fill in the assessment form to clarify the results of disability assessment.
4. Preliminary examination and issuance:
The county-level disabled persons' Federation conducts a preliminary examination according to the relevant materials of the applicant and the disability evaluation results made by hospitals or specialized medical institutions at or above the county level designated by the county-level disabled persons' Federation, and inputs the relevant information of the evaluation form into the basic database of the disabled population.
If the information is false or the disability assessment results made by hospitals or specialized medical institutions at or above the county level designated by the county-level disabled persons' Federation do not meet the disability standards, they shall be returned.
For those who have obvious disability characteristics and can easily identify the category and grade of disability according to the disability standard, the county-level disabled persons' Federation directly fills in the evaluation form, and the disability evaluation results made by hospitals or specialized medical institutions at or above the county level designated by the county-level disabled persons' Federation meet the disability standard, fill in and print the relevant information of the Disabled Persons' Card according to the disability evaluation results, and submit the application form and evaluation form in triplicate to the Municipal Disabled Persons' Federation for approval.
The chairman of the county-level disabled persons' Federation shall sign at the issuer of the disabled card and affix the official seal of the issuing authority in the column of issuing authority.
5. Review, approval and filing:
City Disabled Persons' Federation shall, according to the disability standards and the Measures for the Administration of Disabled Persons' Cards, review the applicant's application for accreditation, the acceptance procedures of county-level disabled persons' federations, the results of disability assessment and the preliminary examination opinions of county-level disabled persons' federations.
Do not meet the disability standards, the county-level CDPF preliminary opinion is wrong or unclear, and other non-conforming returns will not be approved.
Those who meet the disability standards shall be accepted by the county-level disabled persons' Federation, and those whose preliminary opinions are correct and conform to the procedural provisions shall be approved. Stamp the official seal in the column of the examination and approval authority, stamp the portrait of the holder, and keep the application form, evaluation form and other related files.
6. Issuance and filing:
Disabled persons' federations at the county level issue disabled persons' cards recognized by the Municipal Disabled Persons' Federation, and keep relevant files such as application forms and evaluation forms.
7. Receiving and saving:
Applicants go to the county-level Disabled Persons' Federation to receive the approved Disabled Persons' Card, and I keep the application form, evaluation form and other relevant files.
