The examination and approval list xls of the registered change of the second-level constructor (constructor) asks all provincial and central enterprises in Rong to take back (return) the registration certificate and practice seal of the personnel who agree or disagree with the change as soon as possible according to the following requirements.
I. Requirements for collection (return):
1. required information: the applicant's letter of introduction (the trustee's name and ID number must be indicated), and the original and photocopy of the trustee's ID card.
2. Collection time: July 6, 2009-July 2009 10 Monday to Friday morning.
3. Change cost: 65 yuan/piece.
4. Collection place: Window 2, Provincial Construction Talent Exchange Service Hall, 4th floor, No.21Zhimin Road, Chengdu.
5. The application for changing the personnel's practice information card shall be handled by the applicant at Room 806, Office Building, Provincial Construction Department, No.36, Section 4, Renmin South Road, Chengdu (Tel: 028-855333 12).
6. The telephone number for receiving (returning) Chengdu enterprise registration certificate and practicing seal is 86632396.
Second, matters needing attention:
1) The regional column refers to the enterprises in cities, prefectures and counties outside the Provincial Construction Department, so please collect them uniformly from the competent construction departments in the areas listed in the table; If the enterprise needs to collect it by itself, the letter of introduction of its unit shall be stamped with the seal of the local construction department and signed.
2) If the approval opinion is "No Approval", the online information of the unit has been returned, and its registration certificate and practicing seal shall be collected by the enterprise as soon as possible according to the above requirements. On the premise that the badge has not been collected, the second declaration will not be accepted; If it is collected in the future after the expiration, its registration certificate and practice seal will be handled separately. Units belonging to non-provincial construction departments shall be levied by the local construction authorities.
3) As of July 3, 2009, 1-43 batches of first-class construction engineer change approval and 1-43 batches of second-class construction engineer (engineer) change approval can all receive the change registration certificate and practice seal. Applicants who have not yet received it should come and get it as soon as possible.
