Declaration process: (1) uploading materials online. Enterprises log on to the management information service system of "three databases and one platform" of Guangdong Construction Information Network for online declaration, and generate an application form for engineering survey qualification online. At the same time, according to the tips of the qualification application platform, the enterprise uploads the color scanned electronic documents of the attachment application materials to the system online.
(2) Submit the application online. After the online pre-application is approved, the enterprise will directly submit the electronic documents of the application materials (including the electronic documents of the application form and the electronic documents of the attachment application materials) to the Guangzhou Housing and Urban-Rural Development Committee.
(3) receiving results. If the publicity opinion of the enterprise applying for the "three libraries and one platform" is "agreed", the Guangzhou Municipal Commission of Housing and Urban-Rural Development shall make relevant documents after the expiration of the publicity period. If the publicity opinion is "disagreement", the enterprise may file a statement application according to the regulations during the publicity period. For undeclared applications, relevant documents will be produced after the expiration of the publicity period.
The enterprise applying for qualification, after inquiring that the application status is "desirable" on the "three databases and one platform", will take the Notice of Acceptance of Administrative License (which can be printed in the "Operation" column of the application list when logging in to the administrative service platform), the power of attorney of legal person, the ID card of the agent and its copy, and the original qualification certificate (the enterprise applying for qualification newly does not need to submit the qualification certificate) to the office window of Guangzhou Housing and Urban-Rural Development Committee to collect the processing results.
Required information:
(1) To apply for Grade B qualification of engineering investigation for the first time, you need to upload the following materials.
1. Color scanned copies of the original and duplicate business licenses of enterprises as legal persons.
2. Color scanned copy of the identity certificate of the legal representative.
3. Color scanned copies of the identity certificate, graduation certificate and professional title certificate of the main technical person in charge or chief engineer of the enterprise.
4. The identity certificate of the registered practitioners, the color scanned copy of the registration certificate stamped with the practicing seal or the color scanned copy of the registration change form (receipt of acceptance of registration change) or the initial registration form (receipt of acceptance of initial registration) of the provincial registration authority where the enterprise is registered.
5. Color scanned copies of the identity certificate, professional title certificate and graduation certificate of unregistered professional and technical personnel.
6. The main technical person in charge or chief engineer of the enterprise and unregistered professional and technical personnel in the leading specialty shall provide a color scanned copy of personal performance certificate (the certificate shall include the enterprise qualification of the performance completion unit, and the certificate has no specific format requirements), and provide the contact person and office telephone number (fixed telephone number) of the certification unit. Personal performance completed in the reporting unit does not require proof of performance.
7. Color scanned copies of social security certificates (personnel payment details and social security payment vouchers) of major professional and technical personnel (registered and unregistered) in recent 1 month. Among them, professional and technical personnel engaged in engineering design in the military or universities do not need to provide social security certificates, but they need to provide color scanned copies of personnel certification materials from the personnel department of their units.
8. The color scanned copy of the lease contract or loan agreement of the fixed office and the property right certificate of the lessee (borrower) belongs to the color scanned copy of the property right certificate issued by the owner.
9. Color scanned copies of purchase invoices of major technical equipment for engineering investigation.
10. Color scanned copy of legal financial statements of the year before the enterprise applies for qualification or the current period (net assets index must be included).
(2) To apply for upgrading the Grade B qualification of engineering survey, in addition to uploading the materials specified in Item (1), the following materials shall be submitted:
1. original and photocopy of engineering survey qualification certificate, color scanned copy.
2. Color scanned copies of social security certificates of major professional and technical personnel in recent 3 months.
3. The materials in Article 8 (1) need not be provided. In addition, the "registration change form (receipt for accepting change registration) or initial registration form (receipt for accepting initial registration)" mentioned in Article (1) 4 shall not be recognized when applying for upgrading, and the registered personnel must be registered in place.
(three) to apply for the second-level qualification of engineering investigation, in addition to uploading the materials specified in item (a), the following materials shall be submitted:
1. original and photocopy of engineering survey qualification certificate, color scanned copy.
2. Color scanned copies of social security certificates of major professional and technical personnel in recent 3 months.
3. The materials in Article 8 (1) need not be provided. In addition, the "registration change form (receipt of acceptance of change registration) or initial registration form (receipt of acceptance of initial registration)" mentioned in Article (1) 4 shall not be recognized when adding applications, and the registered personnel must be registered in place.
(four) to apply for the extension of Grade B qualification of engineering investigation, the following materials shall be uploaded.
1. Color scanned copies of the original and duplicate business licenses of enterprises as legal persons.
2. Color scanned copy of the original engineering survey qualification certificate.
3. The identity certificate of the registered practitioner and the color scanned copy of the registration certificate stamped with the practicing seal.
4. Color scanned copies of the identity certificate, professional title certificate and graduation certificate of unregistered professional and technical personnel.
5 major professional and technical personnel (registered and unregistered) in recent 3 months. Among them, professional and technical personnel engaged in engineering design in the military or universities do not need to provide social security certificates, but need to provide color scanned copies of personnel certification materials issued by the personnel department of their units.
6. For the main professional and technical personnel (registered and unregistered) who are newly employed (unable to provide the social security certificate for nearly 3 months) and whose personal performance does not belong to the enterprise, the materials in Item (1) shall be provided according to the requirements of applying for enterprise qualification for the first time.
7. Color scanned copies of legal financial statements (including net assets indicators) one year before the enterprise applies for qualification or the current period.
(five) do not meet the simplified examination and approval procedures to re-verify the qualification of engineering investigation, in addition to uploading the materials specified in item (a), the following materials need to be uploaded.
1. original and photocopy of engineering survey qualification certificate, color scanned copy.
2. Color scanned copies of enterprise merger, division, restructuring (reorganization) plan and situation report (including the property rights relationship, capital composition and assets and liabilities of the new enterprise and the original enterprise, the separation and merger of personnel and internal institutions, the separation and merger of engineering survey results, etc.), and relevant resolutions on enterprise merger, division and reorganization (resolutions of shareholders' meeting or shareholders' congress, resolutions of the board of directors, replies from superior administrative departments and state-owned qualification management departments).
3. If an enterprise applies for re-approval, it shall also submit the Approval Form for Change of Qualification Certificate of Construction Enterprise 1 copy.
(six) in line with the simplified examination and approval procedures, please refer to the "guidelines for the reorganization, merger and division of construction enterprises".
Note: 1. Statement by the legal representative of the enterprise. Enterprises applying for qualification by electronic means are required to provide a color scanned copy of the statement of the legal representative of the enterprise (downloaded and printed from the declaration system, signed by the legal representative of the enterprise, responsible for the authenticity of the application materials of the enterprise, stamped with the official seal of the enterprise, and finally uploaded with the color scanned copy). From 2.20 1 July 71day, the insured in the administrative area of Guangzhou may not provide paper social security certificates.
Time limit for completion: make a decision within 20 working days from the date of acceptance (excluding the time for reviewing the enterprise statement and the time for issuing the license documents and certificates by the Guangdong Provincial Department of Housing and Urban-Rural Development).
