This form has two pages. The first page is filled in by the students themselves, and the content must be true and effective. The second page is filled in by the relevant departments of the school. The column of "affiliation" in personal basic information should be filled in according to the actual affiliation level of the school.
5. Fill in the part of the national student loan actually obtained during the school period (only for graduates with loans). National student loan: "Name of loan bank" and "Address of loan bank" please consult the student financial assistance department and fill in; "Loan bank repayment account" refers to the account number filled in the loan contract. Student-origin credit student loan: please fill in the relevant contents according to the signed student-origin loan contract.
6. How to fill in the supplementary fee for me: I need to consult the student independence department of the school for the supplementary fee, and fill it out according to its requirements; The name of the opening bank requires that the name of the opening bank be filled in, not only "China ×× Bank", but also "branch", "savings office" or "credit cooperative".
7.
