Article 1 Composition of an insurance contract:

The China Life Kangning Lifelong critical illness insurance Contract (hereinafter referred to as the Contract) consists of the insurance policy and the attached China Life Kangning Lifelong critical illness insurance Benefit Clause (hereinafter referred to as the Benefit Clause of the Contract), the basic terms of personal insurance (hereinafter referred to as the Basic Terms of the Contract), the cash value table, statements, opinions and approvals, and the insurance application, reinstatement application, health statement and other written agreements related to the Contract.

Article 2 Insurance coverage:

Anyone who is born more than 30 days and under 70 years old and is in good health can be the insured. I or someone with insurance interest can apply for this insurance for the insured.

Article 3 Period of insurance:

The insurance period of this contract is from the effective date of this contract to the termination date of this contract;

Article 4 Major diseases:

The major disease referred to in this contract refers to the disease, disease state or operation of the insured that meets the conditions described in the following definitions. There are 20 diseases, of which the first to seventeenth diseases are listed in the Standard for the Definition and Use of Diseases in Critical Illness Insurance formulated by insurance institute of china, and the rest are diseases added by the company. The names and definitions of major diseases are as follows:

Malignant tumor: refers to diseases in which malignant cells grow and spread uncontrollably, infiltrate and destroy surrounding normal tissues, and can spread and transfer to other parts of the body through blood vessels, lymphatic vessels and body cavities. According to the results of pathological examination, the clinical diagnosis belongs to the category of malignant tumor in the International Statistical Classification of Diseases and Related Health Problems (ICD- 10) of the World Health Organization.

The following diseases are not covered:

1. carcinoma in situ;

2. Chronic lymphoblastic leukemia equivalent to stage A of Binet staging scheme;

3. Hodgkin's disease equivalent to AnnArbor stage I;

4. Skin cancer (excluding malignant melanoma and metastatic skin cancer);

5. prostate cancer 5. T 1n0m 0≤TNM stage;

6. Malignant tumor during 6.HIV infection or AIDS.

2. Acute myocardial infarction: refers to partial myocardial necrosis caused by insufficient blood supply in the corresponding area due to coronary artery obstruction, and must meet at least the following three conditions:

1. Typical clinical manifestations, such as acute chest pain;

2. Recent ECG changes suggest acute myocardial infarction;

3. Myocardial enzymes or troponin increase in diagnosis, or show dynamic changes consistent with acute myocardial infarction;

4. 90 days after the onset, it is confirmed that the left ventricular function is reduced, such as the left ventricular ejection fraction is less than 50%.

3. Sequela of cerebral apoplexy: refers to the sudden changes of cerebrovascular diseases that cause cerebrovascular hemorrhage, embolism or infarction, leading to permanent sexual dysfunction of the nervous system, and refers to permanent sexual dysfunction of the nervous system. This means that after the diagnosis of 180 days, one or more of the following obstacles still exist:

1. One or more limbs completely lose limb function (note1);

2. Complete loss of language ability or chewing and swallowing ability (Note 2);

3. The ability to live independently is completely lost, and it is impossible to independently complete three or more of the six basic activities of daily life (Note 3);

Major organ transplantation or hematopoietic stem cell transplantation: major organ transplantation refers to allograft of kidney, liver, heart or lung due to corresponding organ failure. Hematopoietic stem cell transplantation refers to allogeneic transplantation of hematopoietic stem cells (including bone marrow hematopoietic stem cells, peripheral blood hematopoietic stem cells and umbilical cord blood hematopoietic stem cells) due to hematopoietic dysfunction or malignant tumor of hematopoietic system;

Five, coronary artery bypass grafting (or coronary artery bypass grafting): refers to the actual implementation of open-chest coronary artery bypass grafting for the treatment of severe coronary heart disease. Non-thoracoscopic interventional surgery and endoscopic surgery, such as coronary stent implantation, balloon dilatation of cardiac catheter and laser radio frequency technology, are not covered;

6. End-stage renal disease (or uremia stage of chronic renal failure): refers to chronic irreversible failure of both renal functions, reaching uremia stage, and having received regular dialysis treatment or kidney transplantation for at least 90 days after diagnosis;

7. Multi-limb loss: refers to the complete disconnection of two or more limbs from the wrist or ankle joint (near the trunk end) due to illness or accidental injury;

Eight, acute or subacute severe hepatitis: refers to the diffuse necrosis of liver tissue caused by hepatitis virus infection, leading to acute liver failure, and confirmed by serological or virological examination, and must meet all the following conditions:

1. Severe jaundice or jaundice increases rapidly;

2. Hepatic encephalopathy;

3.b-ultrasound or other imaging examination showed that the liver volume was rapidly shrinking;

4. Progressive deterioration of liver function indicators.

9. Binocular blindness: refers to the permanent and irreversible loss of binocular vision due to illness or accidental injury (Note 4), and the eye with good eyes must meet at least one of the following conditions:

1. Loss or removal of eyeball;

2. The corrected visual acuity is less than 0.02 (international standard visual acuity chart is adopted, and it should be converted if other visual acuity charts are adopted);

3. The visual field radius is less than 5 degrees;

Paralysis: refers to the permanent and complete loss of limb function of two or more limbs due to illness or accidental injury. Permanent complete loss of limb function means that two of the three major joints of each limb are still completely stiff or unable to move with consciousness after 180 days of disease diagnosis or accidental injury 180 days;

Xi。 Severe Alzheimer's disease: refers to the serious decline or loss of intelligence due to progressive and irreversible changes in the brain. Its clinical manifestations are obvious cognitive impairment, abnormal behavior and decreased social ability, and its daily life must be continuously monitored by others. It must be diagnosed by CT, MRI, PET and other imaging examinations, and the ability of independent living is completely lost, and it is impossible to independently complete three or more of the six basic activities of daily life (Note 3);

Neurosis and mental illness are not covered.

Twelve, severe craniocerebral injury: refers to the head suffered from mechanical external force, causing damage to important parts of the brain, leading to permanent dysfunction of the nervous system. The diagnosis must be made by brain tomography (CT), magnetic resonance imaging (MRI) and positron emission tomography (PET). Permanent dysfunction of nervous system means that one or more of the following disorders still exist after 80 days of brain injury/kloc-0:

1. One or more limbs completely lose limb function;

2. complete loss of language ability or chewing and swallowing ability;

3. Completely lost the ability to live independently, unable to independently complete three or more of the six basic daily activities;

Thirteen, severe Parkinson's disease: refers to a degenerative disease of the central nervous system, the clinical manifestations are tremor paralysis, ataxia and so on. , all the following conditions must be met:

1. Drug treatment can't control the disease;

2. The ability to live independently is completely lost, and it is impossible to independently complete three or more of the six basic activities of daily life (Note 3);

Secondary Parkinson's syndrome is not covered;

14. Severe third-degree burn: refers to third-degree burn, and the third-degree burn area reaches more than 20% of the body surface area. The body surface area is calculated according to China's new nine-point method.

Severe motor neuron disease refers to a group of progressive degenerative diseases of motor neurons in the central nervous system, including progressive spinal muscular atrophy, progressive bulbar palsy, primary lateral sclerosis and amyotrophic lateral sclerosis. Must meet the conditions of completely losing the ability to live independently and unable to independently complete three or more of the six basic activities of daily life (Note 3);

Sixteen, severe aplastic anemia: refers to anemia, neutropenia and thrombocytopenia caused by chronic and persistent failure of bone marrow hematopoietic function, and must meet all the following conditions at the same time:

1. The results of bone marrow puncture or bone marrow biopsy support the diagnosis;

2. Peripheral blood pictures must meet the following three conditions:

(1) The absolute value of neutrophils is ≤ 0.5×109/L;

(2) Reticulocyte

(3) The absolute value of platelets is ≤ 20×109/L. ..

17. Aortic surgery: refers to the operation of removing, replacing and repairing damaged aortic vessels through thoracotomy or laparotomy in order to treat aortic diseases. Aorta refers to thoracic aorta and abdominal aorta, excluding branch vessels of thoracic aorta and abdominal aorta.

Intra-arterial angioplasty is not covered;

18. Severe multiple sclerosis: refers to irreversible motor or sensory dysfunction caused by demyelinating diseases of the central nervous system, and its clinical manifestations are visual impairment, paraplegia, imbalance, dysarthria, dysuria and other symptoms. Irreversibility means that motor or sensory dysfunction lasts more than 180 days after the initial diagnosis. The diagnosis must be made by CT, MRI or PET, and all the following conditions must be met:

1. The above clinical symptoms caused by optic nerve, brain stem or spinal cord injury obviously exist;

2. Diverse nerve injuries;

3. The history of clinical symptoms, deterioration and nerve injury of the above-mentioned recurrent attacks;

19. Nephropathy of severe systemic lupus erythematosus: Systemic lupus erythematosus is an immune disease with multiple autoantibodies involving multiple systems and organs. Systemic lupus nephritis, also known as lupus nephritis, involves the kidney and causes renal function damage. The term "severe systemic lupus erythematosus nephritis" as mentioned in this contract refers to type III to type V lupus nephritis that must be confirmed by renal pathological examination or clinical diagnosis and meet the following WHO diagnostic criteria;

World Health Organization (WHO) classification of lupus nephritis;

Type ⅰ (minimal lesion type) was negative under microscope and urine was normal;

Type ⅱ (mesangial lesion) has moderate proteinuria and occasional urinary sediment changes;

Type ⅲ (focal segmental hyperplasia) proteinuria and urinary sediment changes;

Type ⅳ (diffuse proliferative) acute nephritis with urinary sediment changes and/or nephrotic syndrome;

ⅴ type (membranous) nephrotic syndrome or severe proteinuria;

Other types of lupus erythematosus, such as discoid lupus erythematosus and systemic lupus erythematosus involving only other systems such as blood and joints, are not covered;

20. Myasthenia Gravis: refers to an autoimmune disease in which the transmission between nerves and muscle joints is blocked. The clinical feature is that the local or systemic striated muscles are prone to fatigue and weakness, and the extraocular muscles of cranial nerves are the most easily involved, which may also involve respiratory muscles, proximal lower limb muscles and even systemic muscles. All the following conditions must be met:

1. After medical treatment or thymus surgery for more than one year, the patient can't control his illness and lose his normal working ability;

2. There is a critical state of eyelid drooping, or dysarthria caused by involvement of the medulla oblongata muscle, choking when eating, or abnormal respiratory function caused by muscle weakness of the medulla oblongata muscle and respiratory muscle, that is, myasthenia crisis;

3. Alternation of symptom remission, recurrence and deterioration, and clinical treatment history of using anticholinesterase drugs such as neostigmine;

note:

1. complete loss of limb function: refers to two of the three main joints of a limb that are stiff or unable to move with consciousness. Limb refers to the whole upper limb including shoulder joint or the whole lower limb including hip joint;

2. Total loss of language ability or chewing and swallowing ability: Total loss of language ability refers to the inability to make any three of the four sounds (including lip sound, tongue sound, mouth flap sound and throat sound), or the complete removal of vocal cords, or the aphasia due to the injury of brain language center, and the total loss of chewing and swallowing ability refers to the state that chewing and swallowing cannot be carried out due to organic or functional obstacles caused by reasons other than teeth.

3. Six basic activities of daily life: it means (1) dressing: I will dress and undress myself; (2) moving: moving from one room to another; (3) Action: get on and off the bed or get on and off the wheelchair by yourself; (4) Go to the toilet: control yourself to urinate; (5) Eating: take food out of the prepared bowl or plate and put it in the mouth; (6) Bathing: shower or bathe yourself;

4. Permanent irreversibility: refers to the inability to recover by existing medical means after 180 days of active treatment from the date of disease diagnosis or accidental injury;

Article 5 Insurance liability:

During the insurance period of this contract, the Company undertakes the following insurance responsibilities:

The insured suffers from a major disease (regardless of one or more diseases) referred to in this contract for the first time after the effective date of this contract (or the final reinstatement date) 180 days, and the company pays critical illness insurance gold at 300% of the basic insurance amount, and this contract is terminated;

Death insurance: for the insured, our company pays death insurance at 300% of the basic insurance amount, and this contract is terminated;

Article 6 Exemption from liability:

The Company shall not be liable for paying insurance benefits if the insured dies or suffers from serious diseases referred to in this contract due to one of the following circumstances:

1. The applicant intentionally kills or hurts the insured;

2. The insured intentionally injures himself, intentionally commits a crime or resists criminal compulsory measures taken according to law;

3. The insured commits suicide within two years from the date of the establishment of this contract or the final restoration of the effectiveness of the contract, except that the insured commits suicide as a person without civil capacity;

4. The insured smokes, smokes or injects drugs;

Five, the insured drunk driving, driving without a valid driver's license or driving a motor vehicle without a valid driver's license;

6. The insured suffers from a serious disease referred to in this contract or dies within 180 days from the date of entry into force (or final reinstatement) of this contract;

7. War, military conflict, riot or armed rebellion;

Eight, nuclear explosion, nuclear radiation or nuclear pollution;

Nine, hereditary diseases, congenital malformations, malformations or chromosomal abnormalities;

In case of any of the above circumstances, resulting in the death of the insured or serious illness referred to in this contract, this contract is terminated, and the company will refund the cash value of this contract to the insured. If the Insured intentionally kills or injures the Insured, resulting in the death of the Insured, the Company will refund the cash value of this contract and treat it as the legacy of the Insured; If the Insured intentionally kills or injures the Insured, resulting in the Insured suffering from serious diseases referred to in this contract, the Company will refund the cash value of this contract to the Insured;

Article 7 Insurance premium:

The delivery methods of insurance premiums are divided into one-time delivery and installment delivery. If the delivery method is by stages, the payment period is divided into ten years and twenty years, and the delivery method is divided into annual payment and semi-annual payment, which is chosen by the insured when insuring;

Eighth certificates and materials required for insurance;

First, when applying for serious illness insurance, the required documents and materials are:

1. insurance policy;

2. The applicant's legal identity certificate;

3. Diagnostic certificates issued by specialists (including relevant diagnostic basis), medical records, hospitalization and discharge certificates;

4. Other certificates and materials required by the Company that can be provided by the insured to confirm the nature and cause of the insured accident;

Two, when applying for death insurance, the required documents and materials are:

1. insurance policy;

2. The applicant's legal identity certificate;

3. The death certificate of the insured issued by the public security department or the second-class or above (including second-class) hospital;

4. Certificate of cancellation of the insured's household registration;

5. Other certificates and materials required by the Company that can be provided by the insured to confirm the nature and cause of the insured accident;

Article 9 Supplementary Provisions:

If there is any conflict between the basic terms of this contract and the interest terms of this contract, the interest terms of this contract shall prevail;

Article 10 Interpretation:

Accidental injury: refers to the physical injury directly caused by external, sudden, unintentional and non-disease objective events;

Specialist: it means to meet the following four qualifications at the same time: (1) having valid China people's * * * and China's "Doctor Qualification Certificate"; (2) Have a valid practicing certificate of People's Republic of China (PRC) doctors and register with relevant departments on schedule; (3) A valid certificate of the title of physician with the title of attending physician or above in People's Republic of China (PRC); (4) Engaged in clinical work for more than three years in the corresponding departments of hospitals above the second level;

Basic insurance amount: refers to the insurance amount stated in the insurance policy;

Drugs: refer to opium, heroin, methamphetamine (ice), morphine, marijuana, cocaine and other addictive narcotic drugs and psychotropic drugs stipulated in the Criminal Law of People's Republic of China (PRC) and the Criminal Law of People's Republic of China (PRC), but do not include prescription drugs containing drugs prescribed by doctors to treat diseases;

Drunk driving: refers to the fact that when an accident occurs, the alcohol content in the blood of the driver of the vehicle reaches or exceeds a certain standard, and the traffic management department of the public security organ determines it as drunk driving or drunk driving according to the provisions of the Road Traffic Safety Law;

Driving without a valid driver's license: refers to one of the following situations:

(1) Not qualified to drive;

(two) driving a vehicle that does not conform to the driver's license;

(3) Driving with an unqualified driver's license;

(4) When learning to drive with a learner's driver's license, there is no coach to guide the driver, or the driver does not learn to drive according to the prescribed time and route;

No valid driver's license: refers to one of the following situations:

(1) Motor vehicles are deregistered according to law;

(2) Failing to carry out or pass the motor vehicle safety technical inspection on time according to law;

Motor vehicle: refers to a wheeled vehicle driven or towed by a power plant for carrying passengers or articles and carrying out special engineering operations;

War: refers to the armed struggle between countries, ethnic groups and political groups for certain political and economic purposes. Subject to the government announcement;

Military conflict: refers to the armed confrontation between countries or nations within a certain range, subject to the announcement of the government;

Riot: refers to armed riots that disrupt social order, subject to government announcements;

Hereditary disease: refers to the disease caused by mutation or distortion of the genetic material (chromosomes and genes) of germ cells or fertilized eggs, usually manifested as vertical transmission from parents to offspring;

Congenital deformity, deformation or chromosomal abnormality: refers to the deformity, deformation or chromosomal abnormality that the insured is born with. Congenital malformations, malformations and chromosomal abnormalities are determined according to the World Health Organization's International Statistical Classification of Diseases and Related Health Problems (ICD- 10).
