The request should indicate the name of the invention and the name of the inventor , the applicant’s name, address, and other matters.

3. If the patent is not authorized in the current year, a copy of the patent annual fee invoice paid in the current year must be submitted
4. Relevant materials proving that the patented project has been industrialized operation
5. Copies of certificates and related materials awarded for patent projects
