How to fill out the technical expert declaration form

Instructions for filling out the National Technical Expert Application Form

(1) Cover page

1. In the "Name" column, fill in the name used in your ID card and household registration.

2. Fill in the full name of the candidate’s work unit in the “Unit” column.

1. The "date of birth" column should be consistent with the date of birth in the ID card and household register.

2. Fill in the final academic qualifications in the "Education Level" column, such as: graduate school, undergraduate, junior college, technical secondary school, high school, junior high school, elementary school.

3. The "Occupation (type of work) name" column should be the same as the (type of occupation) in the professional qualification certificate.

4. The "Skill Level" column should be consistent with the professional qualification certificate, such as: senior technician, technician, senior worker, intermediate worker, junior worker.

5. The "Working Time" column must be filled in truthfully to the year and month.

6. The "Time engaged in this occupation (type of work)" column must be filled in truthfully to the year and month.

7. The "Work Unit" column is filled in step by step with the name of the head office. It must be consistent with the official seal of the "Recommendations of My Unit" column. Be sure to fill in the full name.

8. Fill in the phone number of my office or workshop in the "Contact number (landline)" column to ensure that I can be contacted (area code required).

9. Fill in your mobile phone number in the "Mobile Phone" column.

10. Fill in your email address in the "E-mail" column.

11. Fill in the "Main Experience" column starting from the highest academic degree obtained, and the starting and ending times must be consecutive.

(3) Second page

1. Fill in the "National Patent Obtained Status" column from front to back in time, and indicate the time, patent name, and patent number in order.

2. "Information on winning scientific and technological progress awards at provincial and ministerial levels or above" should be filled in from first to last in chronological order.

3. In the "Technological Innovation Situation" column, fill in other technological innovation status in addition to the "National Patents Obtained" and "Provincial and Ministerial Level or above Science and Technology Progress Awards" columns.

4. In the "Other unique skills or outstanding contributions" column, in addition to the "Obtained National Patents" and "Awarded Scientific and Technological Progress Awards at or above the provincial or ministerial level" column, there are other special skills or outstanding contributions.

5. The "Awards in Vocational Skills Competition" column should be filled in from high to low in order of competition level, including the awards received in the finals of national first- and second-class or provincial first-class vocational skills competitions.

(4) The third page

1. The "Honorary Titles Received" column should be filled in from the latest honorary title awarded by the provincial, ministerial or industry (won the "National Technology The title of "Expert" needs to be indicated in the way it was obtained, such as in which selection and commendation event or in a certain competition in a certain year.

2. Fill in the "Other Awards" column that is not listed above. Awards at the provincial, ministerial or industry level.

3. Paste a copy of the ID card in the "ID card attachment" column, and the handwriting, numbers, and photos on the copy must be clear.

1. The “Recommendations from My Unit” column shall be signed and stamped by the candidate’s primary work unit

2. Each industry department shall. Recommended candidates, in addition to the opinions and seals of each industry department that must be signed in the "Opinions of the State Council's Industry Competent Departments", all provincial industry departments are also required to sign their opinions in the "Opinions of the Provinces, Autonomous Regions, Municipalities, and Industry Competent Departments" and stamp.

3. Candidates recommended by provinces, autonomous regions, and municipalities, please sign and stamp their opinions in the "Opinions of Provincial, Autonomous Regions, Municipalities, and Labor and Social Security Departments".

4. Candidates recommended by relevant central enterprises are required to sign their opinions in the "Opinions of the Industry Competent Department of the State Council" and affix their official seal at the group company level.
