Legal basis:
Article 121 of the "Civil Procedure Law of the People's Republic of China" shall state the following matters:
(1) The name, gender, age, ethnicity, occupation, workplace, residence, and contact information of the plaintiff, the name, residence, and name, position, and contact information of the legal person or other organization and the legal representative or principal responsible person;
(2) The defendant’s name, gender, work unit, residence, etc., and the name and residence of the legal person or other organization;
(3) The litigation claim and the facts and reasons on which it is based;
(4) Evidence and evidence sources, names and addresses of witnesses.
