(1) Standardize the use of birth medical certificates in birth registration. For newborns who hold the Medical Certificate of Birth, the household registration authority will handle the birth registration after reviewing the Medical Certificate of Birth and other materials. Newborns who can't verify their mother's information can't get birth medical certificates. For newborns who do not meet the conditions for issuance and have not obtained the Medical Certificate of Birth, the household registration authority shall, after investigation and verification, handle the birth registration in accordance with the relevant provisions.

(2) Strict information change. Once the Birth Medical Certificate is issued, the issuing institution will not change the information recorded in the certificate in principle. Where an applicant applies for birth registration to change the name of a newborn, the household registration organ shall register the birth according to the name recorded in the Medical Certificate of Birth, and then go through the formalities of name change in accordance with the relevant provisions, and register the name of the newborn recorded in the Medical Certificate of Birth as the previous name.

(3) Do a good job of authenticity identification. The authenticity appraisal of the "Birth Medical Certificate" shall be accepted and fed back by the county (district) health and family planning administrative department or its entrusted institution where the household registration organ applying for appraisal is located. If the household registration authority finds suspicious "Birth Medical Certificate" during household registration, it will not register the household registration temporarily, and send the suspicious certificate to the local county (district) level health and family planning administrative department or its entrusted institution for authenticity identification. The local county (district) level health and family planning administrative department or its entrusted institution shall verify the authenticity of the certificate carrier and coordinate the county (district) level health and family planning administrative department or its entrusted institution to verify the information recorded in the certificate. The local county (district) level health and family planning administrative department or its entrusted institution shall, after checking the information recorded in the certificate carrier and certificate, issue a written appraisal result and feed it back to the household registration authority. If the level cannot be identified, it can be sent to the first-level health and family planning administrative department or its entrusted institution for identification. When the administrative departments of health and family planning at all levels or their entrusted institutions find false certificates, they shall submit the copies of the certificates and the authenticity appraisal books to the National Health and Family Planning Commission for the record.

(4) Strengthen communication and cooperation. Local health and family planning administrative departments and public security departments at all levels shall promptly forward this notice to management, distribution institutions and household registration organs at all levels. It is necessary to establish a cooperation mechanism, communicate regularly, actively promote the inquiry and verification of the information of the "Birth Medical Certificate" between the two departments, gradually realize the sharing of information on the national network, and do a good job in the opening and management of the new version of the "Birth Medical Certificate".
