In the reporting interface, read the prompts carefully and fill in the corresponding information truthfully. And read the tips at the bottom of the page. After confirming that the information is correct, click Next. If you want to reselect the identity type, click Back. All fields in the information except "E-mail" are required, please fill them in truthfully.
Click Next to enter the "Information Confirmation Page", and then click OK to pop up a prompt box, and click OK to enter the SMS confirmation interface.
Step 5: SMS confirmation
Enter the correct mobile phone number, click get SMS verification code, enter the obtained SMS verification code, click get serial code, enter password, and click submit.
Step 6: Complete the application.
Click Submit, and the system will prompt "Application Successfully", indicating that the data has been submitted to the system background. After completing the data declaration, click the application form to view the application form details, print the application form directly, and click Download to save the application form.
