4. The insured person has personally chosen the community health center. After the insured person is insured in the new unit, the community health center chosen by the insured person is still a designated medical institution for personal medical treatment.
5. If the two or three insured persons of basic medical care change their medical institutions, they will change in the current month and take effect in the next month.
