Article 40 If a motor vehicle driver's license is lost, the driver of the motor vehicle shall apply to the vehicle management office where the motor vehicle driver's license is issued for a replacement. When applying, an application form for a motor vehicle driver's license shall be filled in, and the following documents and vouchers shall be submitted:

(a) the identity certificate of the motor vehicle driver;

Article 41 A motor vehicle driver may entrust an agent to handle the business of replacing or reissuing a motor vehicle driver's license. When an agent applies for a motor vehicle driver's license, it shall submit the identity certificate of the agent and the application form for motor vehicle driver's license signed by the driver and the agent.

The vehicle management office shall record the agent's name, unit name, identity document name, identity document number, residence address, postal code and contact telephone number.
