1. Get and fill in the application form.
To apply for government information disclosure to the accepting institution, an application form for government information disclosure shall be filled out (see the attached table for samples, hereinafter referred to as the application form). A copy of the application form is valid and can be obtained from the accepting institution or downloaded from the website of the government and the health administrative department. Submit an application form
The applicant can fill in the complete application form through the website of the government and the health administrative department, by letter or in person. If you apply by letter, please indicate the words "government information disclosure application" in the lower left corner of the envelope; Applicants can also apply to the accepting institution on site.
The accepting institution does not directly accept the application for government information disclosure made by telephone, but the applicant can consult the application procedure by telephone.
(3) Application processing
After receiving the application, the accepting institution will conduct a formal review of whether the application elements are complete, and return the application with incomplete elements and ask the applicant to make corrections.
If the information applied for belongs to the information that has been voluntarily disclosed by the administrative organ, the accepting institution shall suspend the application acceptance procedure and inform the applicant of the ways and means to obtain the information.
The accepting institution shall handle the applications in the order of receipt. If several independent requests are made in one application at the same time, the accepting institution will give a unified reply after all the requests are processed. In view of the fact that different requests may have different answers, in order to improve the processing efficiency, it is suggested that applicants apply separately for different requests.
Second, the application time limit
After accepting the matters disclosed according to the application according to the workflow, the accepting institution shall give a reply within 15 working days in accordance with the provisions of Article 24 of the Regulations on Information Disclosure in People's Republic of China (PRC).
