(5) The identity certificate, work resume, appointment (employment) documents, graduation certificate, relevant professional education certificate, professional title certificate and registered practice certificate of the registered supervision engineer stamped with the practice seal;

6. The identity certificate of the registered practitioners listed in the Application Form for Qualification of Engineering Supervision Enterprises and the copy of the registered practice certificate stamped with the practice seal (the new enterprise applying for qualification may not be stamped with the practice seal);

(7) Proof of office space of the enterprise (provide lease contract and lessor's property right certificate for more than one year; Self-owned should provide proof of housing property rights);

(8) Necessary procurement list of engineering test and testing equipment (filled in according to the requirements of the application form, with an invoice attached);

Pet-name ruby enterprise management system certification materials: personnel, finance, supervision system and other management systems;

(3) The original and photocopy of the qualification certificate of first-class communication engineering.
