According to the relevant requirements of the Provincial Department of Education and the Notice of the Provincial Teacher Qualification Management Office on Doing a Good Job in the Qualification Certification of Primary and Secondary School Teachers in our Province in 20021year, and in combination with the relevant work arrangements of the Wuhan Municipal Education Bureau, the relevant matters concerning the qualification certification of junior high schools and below in our district in the spring of 20021year are hereby announced as follows:
I. Object and scope of appraisal
China citizens who have not reached the national statutory retirement age, whose household registration and residence (they must hold a local residence permit and be within the validity period) or school location (only full-time college graduates and full-time graduate students in 20021year) are in Wuhan Donghu New Technology Development Zone, as well as active servicemen and armed police stationed in this area, who have obtained the qualification certificate of primary and secondary school teachers' qualification examination and are within the validity period or meet the recognition conditions, can apply for the recognition of junior high schools, primary schools and kindergartens in our district.
According to the general office of the Ministry of Education? Secretariat of the Central Taiwan Province Provincial Work Office? The Notice of the Secretariat of the the State Council Hong Kong and Macao Affairs Office on Issues Related to the Application of Hong Kong, Macao and Taiwan Residents for the Qualification of Primary and Secondary School Teachers in the Mainland (Teacher's Office [2065 438+09] 1No.) stipulates that Hong Kong, Macao and Taiwan residents who study, work and live in our district have no criminal record, and can apply for recognition at their place of residence with valid certificates such as residence permit for Hong Kong, Macao and Taiwan residents or five-year mainland pass for Taiwan Province residents. The academic qualifications, other conditions and procedural requirements for applying for teacher qualification are the same as those for mainland (mainland) applicants. The certificate of no criminal record issued by Hong Kong, Macao and Taiwan residents is provided by the provincial teacher qualification management office.
Second, distinguish the time and object of online reporting.
This spring, we will register twice. The specific agenda and target audience are as follows:
June1-June 6 is the system maintenance time, and the website access and user confirmation functions are closed.
III. Relevant conditions and standards
(a) the competent authority. The qualifications of kindergarten, primary school and junior high school teachers are recognized by the education administrative department at the county (district) level; The qualifications of practice instructors in senior middle schools, secondary vocational schools and secondary vocational schools shall be examined by the education administrative department at the county (district) level where the applicant's household registration is located and reported to the education administrative department at the next higher level for confirmation.
(2) academic standards. The application for the qualification of primary and junior high school teachers shall be implemented in accordance with the relevant provisions of the Work Instructions for the Qualification of Teachers in Our Province (E.J.Z. [2012] No.4), and the application for the qualification of kindergarten teachers shall be implemented in accordance with the relevant provisions of the Notice on the Application for the Qualification of Kindergarten Teachers in Our Province (E.J.Z. [2015] No.6).
(3) Putonghua requirements. To apply for the qualifications of primary school general practitioners, Chinese subjects and kindergarten teachers, you must reach the second-class and above standards. To apply for other teacher qualifications, you must meet the second-class and above standards.
(4) Physical examination standard. In strict accordance with the "Hubei Province to apply for teacher qualification physical examination standards and methods" (Hubei teacher hair [2002] No.3) provisions. The physical examination of the personnel applying for kindergarten teacher qualification shall be carried out in accordance with the provisions of the Notice of the Ministry of Education on Adjusting the Physical Examination Standards of the personnel applying for kindergarten teacher qualification. All physical examination items must be completed.
The medical examination time is valid for 2 months from the date of this announcement; The applicant chooses a hospital for physical examination from the list of hospitals designated for physical examination of teacher qualification in Wuhan (see Annex 1), and uses the physical examination form of the applicant for teacher qualification in Hubei Province. All physical examination items must be completed. The provincial people's hospital is the final inspection hospital of doubt.
Note: The date of the second physical examination of the online newspaper personnel shall not be earlier than April 29th to ensure the validity of the physical examination results.
Four, the teacher qualification steps
(1) Register online. Applicants should log on to China Teacher Qualification Network (www.jszg.edu.cn) within the specified time and declare from the "Teacher Qualification Applicant Portal". First, register the application account with your real name, then complete the verification or supplement of Putonghua proficiency test grade certificate, academic certificate, degree certificate and teacher qualification examination information on the personal information page, download, print, sign and upload personal books, select accreditation institutions and on-site confirmation points, fill in the confirmation declaration information, and submit the accreditation application.
When filling in online, please choose the type of application accurately: social workers choose the domicile and residence permit, and full-time college graduates and full-time graduate students choose the location of the school; Accreditation body: For those who apply for the qualification of teachers in junior high schools, primary schools and kindergartens, the Education Bureau of Wuhan Donghu New Technology Development Zone shall be selected as the accreditation body; On-site confirmation point: ① social personnel with household registration and valid residence permit in our district choose the window of the Education Bureau of Wuhan Donghu New Technology Development Zone as the on-site confirmation point; ② Full-time college graduates and full-time graduate students choose their college as the on-site confirmation point (provided that the college applies to the District Education Bureau to open the school confirmation point before April 12).
(2) On-site confirmation. The applicant shall, within the specified time, bring the specified materials to.
The on-site confirmation points selected during the webcast will participate in the on-site audit and confirmation, and those who fail to participate in the on-site confirmation at the specified time and place will be deemed to have given up automatically.
The first on-site confirmation time for social workers: May19-May 25th (9: 00 a.m.-65438+ 2: 00 p.m., 65438+0: 30-5: 00 p.m., in which on-site confirmation is not accepted on 22nd and 23rd).
The second on-site confirmation time for social workers: working hours from June 23rd to June 29th (9: 00 am-12: 00 am and 1: 30-5: 00 pm, in which on-site confirmation is not accepted on 26th and 27th).

(4) Issuing certificates. Applicants who apply for high school teacher qualification can check the announcement on the website of Wuhan Education Bureau, and applicants who apply for junior high school and below can check the announcement on the government affairs website of Wuhan Donghu High-tech Zone. Applicants who choose "one-time handling" will receive the certificate by postal express (postage collected); Applicants who do not choose "one-time job" shall apply for the qualification certificate of senior high school, junior high school and below at window D23, 2nd floor, Government Affairs Center, Wuhan Donghu High-tech Zone. "Teacher Qualification Certificate" and "Accreditation Application Form" (referred to as "one certificate and one form") are collected or mailed at the same time. Please pay attention to sign for it.

(A) according to the provincial teacher qualification center "on further standardizing the higher education in our province
According to the spirit of the Notice on the Accreditation of Teachers' Qualifications for Fresh Graduates from Schools and Secondary Normal Schools (E.J.Z. [20 10] 1No.), ordinary full-time fresh graduates from Donghu High-tech Zone and full-time graduate students from colleges and universities should be organized by their schools to apply to the teacher qualification accreditation institutions where the schools are located, and the District Education Bureau will not accept individual applications from fresh graduates. Colleges and universities that need to organize 202 1 ordinary full-time graduates and full-time graduate students to apply for junior high school and below teacher qualifications, please write to our bureau before April 15 to apply for opening on-site confirmation points, submit work plans as required, and do a good job in on-site confirmation and material review. If colleges and universities fail to apply for opening on-site confirmation points within the time limit, or fail to conduct on-site confirmation and material review as required, the District Education Bureau will no longer accept their application for teacher qualification.
(2) upload personal photos and books correctly. Photos uploaded by the information system and photos pasted by teachers' qualification certificate holders use recent bareheaded 1 inch color photos with white background (the upload format is JPG/JPEG, which is no more than 200K). After the online report is successful, the applicant must preview his Appraisal Application Form generated by the system. If the account books and photos are uploaded incorrectly, it will affect the generation of the approved Appraisal Application Form. The personal signature on the book should be written in print. In particular, the applicant should check: ① whether the photo is my own and clear; (2) Whether the book is intercepted according to the border range, whether the upload is upright, whether the handwritten signature is for me, and whether the time is complete; If the preview finds that the uploading location of the personal book is incorrect or unclear, or the signature is incomplete or unclear, you should upload it again. After the preview is correct, print the teacher qualification application form 1 copy generated by the system through the browser printing function, and submit it to the site for confirmation.
(3) After the online application is completed, the applicant shall carry the following materials for on-site examination and confirmation at the specified time and place:
1. Print the system-generated teacher qualification application form 1 copy on A4 paper;
2 "Teacher Qualification Applicant Information Checklist" in duplicate;
3. Original second-generation ID card (within the validity period).
4. Original academic certificate. Hong Kong, Macao and Taiwan academic degrees should also be submitted to the original certificate of Hong Kong, Macao and Taiwan academic degrees issued by the study abroad service center of the Ministry of Education, and the original certificate of foreign academic degrees issued by the study abroad service center of the Ministry of Education should also be submitted. Academic information may not be submitted if it is verified by electronic information in the online newspaper system.
Special note: in the process of material review, applicants must submit the China Higher Education Qualification Report (online application on Xuexin.com(www. chsi. com. cn)) for the academic qualifications that cannot be directly compared and verified by the national information system (except secondary vocational school qualifications), otherwise it will not be accepted and will be regarded as unqualified academic qualifications. Applicants are advised to verify their academic qualifications in Xue Xin. If it cannot be verified, please apply for a certification report in time.
5.？ The original physical examination form of the applicant for teacher qualification in Hubei Province whose physical examination conclusion is "qualified" (if the prescribed physical examination form is not used, the physical examination will be invalid if the applicant is not in the designated hospital and within the specified time period).
6. Original certificate of Putonghua proficiency test level (if the electronic information of the online newspaper system has passed the verification and meets the requirements of Putonghua proficiency level in the Measures for the Implementation of Teachers' Qualification Regulations and the Language and Character Law, it may not be submitted).
7. Servicemen and armed police who apply at the place of service shall provide military or police officers' certificates.
The official certificate, if the service place cannot be shown on the certificate, also needs to provide the personnel relationship certificate issued by the subordinate unit or the personnel department of the unit organization. The format of the certificate depends on the regulations of the unit or unit, and the certificate should clearly indicate the applicant's service place;
8. Examination certificate or "Teachers' Professional Ability Certificate of Normal School Students". Through the electronic information verification of the online newspaper system, full-time normal education graduates who entered school in 20 1 1 year or earlier need not submit.
9. Recent bareheaded front 1 inch color white background photo (official photo, used to apply for teacher qualification certificate, should be the same as the photo uploaded when applying online, with name and ID number indicated on the back of the photo).
10. If you apply for confirmation at the place where your household registration is located, submit the original household registration book or collective household registration certificate; To apply for recognition in the place of residence, the original valid residence permit shall be submitted; If the applicant applies for accreditation based on the location of the school, he shall submit the original student ID card or academic certificate with complete registration information (limited to 202 1 fresh graduates and full-time graduate students of full-time colleges and universities in China). Hong Kong, Macao and Taiwan residents are required to submit their residence permit for Hong Kong, Macao and Taiwan residents, or their travel permit to and from the mainland or the original valid documents such as mainland travel permit for taiwan residents valid for five years.
(4) If the applicant is a full-time graduate majoring in normal education who enrolled 20 1 1 years ago, in addition to submitting the materials according to note (3), he must also provide the original and photocopy of his compulsory course scores in pedagogy and psychology and his educational practice certificate (the photocopy is in his personnel file), and the photocopy of the applicant's information page in the college admission roster (both must be stamped with the seal of the file keeping institution, indicating that it is checked correctly with the original).
(5) Applicants can only apply for accreditation and obtain teacher qualification certificate every year. Please make online declaration and on-site confirmation according to the specified time, place and requirements. If the applicant fails to complete the application or supplementary materials within the specified time due to reasons such as missing the application time, choosing the wrong accreditation institution or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation institution will no longer accept it; When reporting online, the applicant should carefully read the description of the online newspaper, register and authenticate the real name as required, and fill in the information truthfully and accurately. The platform cannot be verified due to false positives or concealment, and the consequences caused shall be borne by the applicant.
(6) After the ideological and moral appraisal form is replaced by a personal book, the applicant has no proof of criminal record, and according to the principle of territorial management, the District Education Bureau will send a letter to the public security organ in its jurisdiction for background check after completing the on-site confirmation; Full-time 202 1 fresh graduates and full-time graduate students collectively issue ideological and moral certificates, with a list (student name, gender, ID number, registration number, college, etc. ) Combined with students' performance in school. Applicants who commit acts of dishonesty, such as practicing fraud and defrauding teachers' qualifications, or have criminal records after being recognized by public security organs will be blacklisted and punished according to law, such as joint punishment for dishonesty and "revocation" and "loss" of teachers' qualifications. If a teacher's qualification is revoked, he/she shall not regain his/her teacher's qualification within 5 years from the date of revocation; Those who lose their teacher qualifications may not re-apply for teacher qualifications.
(seven) the teacher qualification applicant information checklist is the basis of the identification process. After downloading this document, the applicant should fill in the information by computer (manual filling is not recommended), and print and paste the printed photos and handwritten signatures on A4 paper.
