Second, the application procedure
This application procedure can choose two modes: online registration+on-site appraisal and online registration+online appraisal. The specific process is as follows:
(1) Online registration+on-site appraisal
1. The applicant registered in the "Teacher Qualification Website Portal" of China Teacher Qualification Network (www.jszg.edu.cn) from May 17 to June 20 (7:00- 17:00).
Among them, 202 1 education graduate students and public-funded normal students who hold the Certificate of Teachers' Professional Ability for Normal Students began to register after May 10.
2. From May 65438 to June 25 (working day), I went to Deyang Fifth People's Hospital (formerly Dongdian Hospital) for an fasting physical examination.
3. From June 25th to July 9th, log on to Sichuan Government Affairs Service Network () and upload my ID card and teacher qualification application form in picture or PDF format (exported after successful online registration of national teacher qualification).

4. From June 28th to July 9th (working day), go to No.73, the third floor of Deyang Municipal Affairs Service Center (next to Wuzhou Square) for on-site confirmation.
Please bring the following materials when confirming on site:
① Original second-generation ID card (within the validity period).
(2) Original academic certificate (China Teacher Qualification Network system can be verified successfully without submission). Hong Kong, Macao and Taiwan academic degrees should also be submitted to the original certificate of Hong Kong, Macao and Taiwan academic degrees issued by the study abroad service center of the Ministry of Education, and the original certificate of foreign academic degrees issued by the study abroad service center of the Ministry of Education should also be submitted.
Special note: In the process of material review, applicants must submit the China Higher Education Qualification Report (Xuexin.com Online Application) for academic qualifications that cannot be directly compared and verified by the national information system (except secondary vocational school qualifications), otherwise they will not be treated as unqualified academic qualifications. (3) Physical Examination Form for Applicants with Teacher Qualification in Sichuan Province (valid for one year).
(4) Original Putonghua proficiency test grade certificate (China Teacher Qualification Network system can be verified successfully without submission).
⑤ Certificate of primary and secondary school teachers' qualification examination within the validity period (please print it on the National Primary and Secondary School Teachers' Qualification Examination Network (ntce.neea.edu.cn) by yourself) or certificate of teachers' professional ability of normal students (limited to 202 1 education graduate students and public-funded normal students).
Affected by the epidemic in COVID-19, the validity of the Certificate for Primary and Secondary School Teachers, which expired on June 30th, 2020 and February 3rd, 2020, was extended for one year.

⑦ Household registration certificate or residence certificate. To apply for recognition in the place of residence, the original valid residence permit shall be submitted.
