There are generally two ways to unbind family members from medical insurance cards:
1. Go to the local social security bureau to go through the formalities of unbinding. I need to bring my ID card, medical insurance card, family doctor signing agreement and other related materials to the social security bureau window to untie;
2. Unbind at the bank counter. Family members need to trust each other, which is only applicable to the case that family members withhold and remit medical insurance premiums through banks. When unbinding, you need to bring your ID card and medical insurance card, fill in the relevant unbinding application form and other materials, and go to the bank counter to handle it.
Materials required for medical insurance card:
1. Two recent two-inch bareheaded photos of the insured. If the insured is a preschool child, provide two recent two-inch bareheaded photos of mother and son or father and son;
2. The original and two copies of the insured's ID card;
3. The original and two copies of the household registration book of the insured unit, and one copy of each page of the household registration book;
4. The original and photocopy of the valid certificate and other relevant materials of other family members except the insured person in the household registration book; For example, family members have medical insurance books of provinces and cities. You just need to bring the original medical insurance book and copy the first page of the medical insurance book.
5. When the children of migrant workers are insured, they also need to provide the original and photocopy of their parents' temporary residence permit, the original and photocopy of their household registration book, the proof of going out to work provided by their parents' unit and the proof of labor relations for more than 3 years;
6, belonging to the minimum living allowance, but also need to provide the minimum living allowance certificate issued by the civil affairs department and the original and photocopy of the registration materials for receiving the minimum living allowance in that year;
7, is a severely disabled (one to two disabled) personnel, but also need to provide the disabled Federation issued the "disabled card" and a copy of the original;
8. If a low-income family is over 60 years old, it is also necessary to provide relevant certification materials issued by the civil affairs department for the elderly in low-income families.
To sum up, the employer shall, within 30 days from the date of employment, apply to the social insurance agency for social insurance registration for its employees. If the social insurance has not been registered, the social insurance agency shall verify the social insurance premium it should pay. Employees-free individual industrial and commercial households who voluntarily participate in social insurance, part-time employees who do not participate in social insurance in the employing unit and other flexible employees shall apply to the social insurance agency for social insurance registration.
