Materials to be provided:
1, Application Form for Motor Vehicle Driving License;
2. Physical condition certificate issued by the medical institution;
3, motor vehicle driver's identity certificate (see notice fifteen) and a copy.
The renewal and verification of a driver's license shall meet the following conditions:
(a) holding a driver's license of a large bus, tractor, city bus, medium-sized bus or large truck without scoring in this scoring cycle, or holding other driving licenses without scoring 12 in this scoring cycle;
(2) Drivers who hold driver's licenses for large buses, tractors, city buses, medium-sized buses and large trucks and score points in a scoring cycle, and those who hold other quasi-driving driver's licenses and bear the same or more responsibilities for causing death in traffic accidents, and whose motor vehicle driver's licenses have not been revoked, have participated in certification education;
(3) The applicant has no unfinished road traffic safety violations or traffic accidents;
(4) The applicant's physical condition meets the driver's license conditions;
(5) The motor vehicle driver's license has not been sealed up, detained, temporarily suspended, revoked, cancelled or revoked according to law.
The driver's license is about to expire, and the following replacement procedures are required:
1, the identity certificate of the motor vehicle driver;
2. Motor vehicle driver's license;
3. A physical condition certificate issued by a medical institution at or above the county level or a medical institution at the regiment level;
4, belongs to the application for the disabled special small automatic passenger car, submitted by the provincial health authorities designated by the specialized medical institutions issued by the relevant physical conditions.

Motor vehicle drivers shall, within 90 days before the expiration of the motor vehicle driver's license, apply to the motor vehicle management office at or outside the place where the motor vehicle driver's license is issued. The application information shall be confirmed when applying, and the following certificates and vouchers shall be submitted:
(a) the identity certificate of the motor vehicle driver;
(2) The physical condition certificate issued by the medical institution.
