According to the Announcement of Xuancheng Municipal Organs 202 1 Open Selection of Civil Servants, the relevant matters of physical examination are hereby notified as follows:
I. Time and place of physical examination
1. Physical examination time: 202165438+February 25th (Saturday).
2. Registration place: Candidates must report to the designated place on an empty stomach before 07: 0015 on the day of the medical examination, and participate in the medical examination according to the regulations and requirements (see Xuancheng 202 1 Announcement on Public Selection of Civil Servants for details).
Second, get a medical notice.
1. Solicitation method: online solicitation method. Specifically, candidates use IE browser, log on to the website of 202 1 Xuancheng Municipal Government's Public Selection of Civil Servants (see attachment 1), and after paying the medical examination fee, print "Xuancheng Municipal Government's Public Selection of Civil Servants 202 1" on the Internet by themselves.
2. Payment method and fee standard: Candidates pay the medical examination fee of 470 yuan/person (including transportation fee) through online payment tools.
3. Payment printing time: 202165438+Thursday, February 23rd to Friday, February 24th.
Candidates who have not applied for online payment and received the medical examination notice within the specified time are regarded as automatically giving up the medical examination qualification and no longer being affiliated.
Third, fill in the physical examination confirmation form and health book.
Candidates download and print the Confirmation Form for Public Selection of Civil Servants by Xuancheng Municipal Authorities 202 1 and the Health Book (see Annexes 2 and 3), and fill in and sign with a black pen or signature pen. One-inch bareheaded photos with the same background submitted during online registration (that is, photos with the same background as the written test admission ticket) are posted in the "photos" section. Fill in the form with clear handwriting, no alteration, truthfully fill in the medical history item by item, without omission.
Candidates are requested to submit a complete Confirmation Form for Public Selection of Civil Servants by Xuancheng Municipal Authorities (202 1) and a health booklet when they report for duty on the day of physical examination.
Fourth, medical considerations.
1. Documents required for physical examination. According to the regulations, candidates must hold the medical examination notice, written examination admission ticket (which can be printed from the registration website), their valid resident ID card (original) and nucleic acid test report within 48 hours before the medical examination.
2. Epidemic prevention and control requirements. According to the requirements of epidemic prevention and control in COVID-19, candidates who have lived in the city where the high-risk area is located 14 days before the physical examination or whose "health code" is not green code are not allowed to participate in the physical examination. If you have fever, fatigue, cough, dyspnea, diarrhea and other symptoms before physical examination, please truthfully report to the local disease control department and see a doctor in time. Those who feel unwell during the physical examination should report to the staff immediately and obey the management of the staff. Anyone who conceals or lies about the key information of epidemic prevention and control, such as residence history, contact history and health status, and does not cooperate with the staff to carry out epidemic prevention testing and inquiry, resulting in adverse consequences, shall terminate his physical examination and be investigated for legal responsibility according to law.
3. Other precautions. Please read the physical examination instructions carefully (see Annex 4). Personal information shall not be disclosed during the medical examination, otherwise it will be dealt with in violation of discipline.
