(2) Application procedure: submit the following materials at one time:
1, submit a copy of work-related injury confirmation issued by the work-related injury identification institution or a power of attorney for work-related injury identification;
2, fill in the "Xiamen city industrial injury workers lose the ability to work appraisal form" a (with a one-inch photo, the official seal of the employer);
3. Submit the Clinical Physical Examination and Diagnosis Certificate of Medical Termination for Workers with Work-related Injury (with the official seal of the hospital) and attach the inspection reports of the hospital;
4. Copies of all medical records, physical examination reports, diagnosis certificates and other materials of the employees' work-related injuries and their ID cards;
5. Patients with occupational diseases should provide the occupational disease diagnosis conclusion issued by the Occupational Disease Diagnosis Group of the Municipal Center for Disease Control and Prevention;
6, physical disability, burns and other surface disabilities, should be submitted to the disabled part of the four-inch color photos;
7. Pay the appraisal fee;
(3) Identification time
1, in line with the medical termination standards and medical termination period, generally within 60 days after the identification materials are submitted, the municipal labor appraisal office.
Arrange the evaluation. Special circumstances can be extended for another 30 days to make an appraisal conclusion.
2, due to fracture, joint injury, nerve injury and other medical end period is not full, rehabilitation? If it takes time, burns, disfigurement and other special circumstances, and it is necessary to wait for the expiration of medical treatment or recovery, the appraisal time will be notified separately, and the injured workers will be asked to wait for the notice.
(4) Organization identification
1. Implement face-to-face assessment system. The municipal labor inspection office informs the appraiser to go to the designated place and accept the clinical examination and appraisal by the experts of the appraisal group. The appraiser must carry X-ray films, ct films and various inspection reports for future reference.
2. Each appraisal shall be attended by more than three experts of the appraisal group, and the appraisal result shall be made and signed by three experts.
3. When the municipal labor inspection organ deems it necessary, it can organize verification or social investigation within 7 days after the expert appraisal, and go to the unit, residence or hospital where the appraiser is located to investigate and verify the situation.
(5) Evaluation results
1, the appraisal result will be made within 7 days after the appraisal, and the municipal labor appraisal office will notify the appraiser unit and the appraiser himself to receive it.
2, according to the national standard (gb/t 16 180? 1996) Identification of the degree of injury and occupational disease disability. The appraisal result is divided into 10 grade, in which 1 is disabled. Is level 4 completely incapacitated and level 5 disabled? Grade 6 means that most of them lose their ability to work and are disabled at grade 7? 10 is a partial loss of working ability.
(6) Re-appraisal
1, the units and individuals applying for appraisal have objections to the conclusion of the city's labor ability appraisal, and may put forward a re-appraisal to the provincial labor appraisal committee within 15 days after receiving the appraisal book. The appraisal result of the Provincial Labor Appraisal Committee is the final conclusion.
(seven) review and appraisal: since the conclusion of labor ability appraisal is made 1 year later, the employee himself or his immediate family members and the institute.
If the unit or agency thinks that the disability situation has changed, it may apply for re-examination and appraisal of labor ability.
(8) Precautions
1. Please come here in the morning to handle the above matters, and arrange the appraisal and internal business in the afternoon. Reception is not allowed.
2. Regarding the treatment of work-related injuries, please consult the Medical Work-related Injury Department or the Work-related Injury Insurance Rehabilitation Department of the Municipal Social Insurance Management Center.

4.
