Several situations of hepatitis B infection:
1)HBsAg positive, HBV DNA negative, transaminase continued to be normal, regardless of HBeAg positive or not. Known as inactive HBsAg carriers. No antiviral treatment is needed.
2)HBsAg positive, HBV DNA positive (often high copy number), transaminase continues to be normal, regardless of HBeAg positive or not. Known as HBV Airlines. There is no need for antiviral treatment for the time being.
3)HBsAg is positive, HBV DNA is positive, transaminase is abnormal repeatedly, and HBeAg is positive. It is called HBeAg-positive chronic hepatitis B and needs antiviral treatment.
4)HBsAg is positive, HBV DNA is positive (often with high copy number), transaminase is abnormal repeatedly, and HBeAg is negative. Called: HBeAg negative chronic hepatitis B antiviral treatment should be more active. Moreover, the course of antiviral treatment needs to be longer, and the monitoring during medication should be stricter. Because these patients are more prone to relapse and drug resistance.
5) If there is liver cirrhosis and HBV DNA is positive, active antiviral treatment is needed regardless of transaminase.
At present, according to statistics, 1-2% of HBV-infected people turn negative spontaneously every year. However, the purpose of drug therapy is to "inhibit virus replication to the greatest extent, inhibit hepatocyte inflammation for the longest time, and then stop the process of liver fibrosis and prevent the occurrence of adverse disease outcomes such as cirrhosis and liver cancer." No drug is aimed at "successful negative HBsAg". However, many patients with chronic hepatitis B can turn sAg negative during long-term effective antiviral treatment.
