Notice on the change of the name of the practicing unit of the 20 12 (Phase I) registration certificate of consulting engineer (investment)
Relevant engineering consulting units:
If the company name is changed before 20 1 1 and 12, and you have received the qualification certificate of the renamed engineering consulting company, please change the name of the practicing company as soon as possible with the registered consulting engineer (investment) registration certificate from now until March 6. Where the qualification certificate of unit engineering consulting has been changed to a new name, and the practicing unit in the registration certificate of registered consulting engineer (investment) is still the old name, please contact the provincial engineering consulting association as soon as possible to apply for change declaration. The following 1-6 written materials shall be submitted when going through the alteration formalities, of which 1-5 is in duplicate.
1, unit application;
2. The qualification certificate of the renamed engineering consulting unit (original and photocopy);
3. Renamed business license of enterprise legal person or certificate of institution legal person (copy);
4. A copy of the approval notice of name change issued by the industrial and commercial department or the approval of the competent department for name change;
5. List of registered consulting engineers (investors) who need to change the company name (including Excel forms, sent to ths1952 @163.com);
6. Original registration certificate.
