(3) Handling procedures
1. The outpatient registration window or self-service registration machine hangs the special disease outpatient number (on-site handling); 2. Prepare the original and photocopy of the ID card or medical insurance card and medical record, and wait for the station call; 3. According to the call number, go to the designated window in turn to issue a special disease diagnosis certificate; 4. Register for consultation at the designated window; 5. Go to the designated window for review and seal; 6. Carry the diagnosis certificate of special diseases and related materials to the medical insurance bureau of the insured districts and counties for special disease certificates.
(4) Materials to be provided
1. Inpatients should provide copies of medical records of hospitals at or above the second level related to the declared diseases, including home pages, admission records, discharge records, operation records, pathological reports, tumor marker test reports (such as alpha-fetoprotein and prostate-specific antigen), imaging test reports (such as CT, PET-CT, magnetic resonance imaging and bone scanning) and other related materials. All materials need to be stamped with the special seal of the hospital. 2. Only outpatients who are not hospitalized need to provide the original outpatient medical records related to the declared diseases (signed and sealed by the doctor) and the inspection data of secondary and above hospitals (the requirements are the same as above). 3.CT, PET-CT, magnetic resonance imaging, bone scanning and other imaging films (except those with definite pathological diagnosis report and no distant metastasis). 4. The original and photocopy of the patient's ID card or medical insurance card.
(5) Remarks
Medical records that have been applied for more than five years must be re-examined. Only those with recurrence or metastasis can be treated, and the original medical records, re-examination medical records and examination data must be submitted when handling.
