Article 1 The Provincial Department of Human Resources and Social Security or the Municipal Department of Human Resources and Social Security may issue social security cards with the approval of Ministry of Human Resources and Social Security. No other institution or organization may issue social security cards.
2. Article 2 The areas where social security cards are issued (hereinafter referred to as card-issuing areas) shall establish standardized human resources and social security business processes with strong comprehensive coordination among businesses, which can ensure the effective application of social security cards. At the same time, it shall also meet the following technical conditions:
(a) to establish a business management system, database and information network suitable for the use of social security cards;
(two) have the technical force to support the management and application of social security cards, including personnel and equipment, and be able to quickly complete the system layout of social security card applications;
(3) Formulating standardized and feasible implementation plans, including application design plans, cost solutions, information collection plans and specific distribution plans;
(four) to establish a scientific and perfect social security card issuance management system and clear internal control procedures, and formulate emergency plans;
(5) Other conditions stipulated by Ministry of Human Resources and Social Security.
Article 3 To apply for issuing social security cards, the following materials shall be submitted:
(1) Fill in a complete social security card issuance registration application form;
(2) An explanation of the preparation for the issuance of social security cards;
(3) The application fields and application file structures in the card that meet the requirements of the national unified specifications (including the application fields and indicators of local expansion);
(four) the card face style designed according to the unified requirements;
(five) the measures for the administration of social security cards in this area and the detailed rules for their implementation;
(six) other materials required by Ministry of Human Resources and Social Security.
4. Article 4: Procedures for registration and approval of social security cards:
(1) When issuing social security cards, the provincial human resources and social security departments shall submit the application materials to the office of Ministry of Human Resources and Social Security Informatization Leading Group for review.
(two) municipal human resources and social security departments to issue social security cards, the application materials should be submitted to the provincial human resources and social security departments for preliminary examination. After passing the preliminary examination, it shall be reported to the Office of Ministry of Human Resources and Social Security Informatization Leading Group for review.
(3) With the approval of the Office of the Information Leading Group of Ministry of Human Resources and Social Security, the identification numbers of the social security card issuers are uniformly distributed to the areas that have approved the issuance of social security cards, and shall be filed with the National IC Card Registration Center in accordance with the relevant provisions of the state.
(four) in the card-issuing area, if the structure of the social security card surface and the file in the card is changed, it must be reported to the Office of the Information Leading Group of Ministry of Human Resources and Social Security for re-examination. Any change of card-issuing suppliers and products, expansion of card-issuing quantity and increase of card-issuing quantity shall be reported to the Office of Ministry of Human Resources and Social Security Informatization Leading Group for the record.
