Education: the candidate's highest education before obtaining the qualification.
Pharmacy, traditional Chinese medicine, clinical and other majors.
Graduation time: the graduation time of candidates with the highest academic qualifications before taking this qualification examination.
The relevant professional years before obtaining this qualification, candidates have been engaged in relevant professional work, and there is no need to fill in.
Years of work in this major before qualification.
Candidates with technical positions in their own units, such as junior (middle) pharmacists and junior (middle) pharmacists, need not fill in those that are not available.
Appointment time Before obtaining this qualification, it is not required that the applicant be appointed as a professional and technical position by the unit. ?
What are the technical qualifications of candidates before taking this qualification? For example, if the candidate has obtained a primary pharmacist, you can fill in "Primary Pharmacist" in this column, otherwise it is not necessary.
Candidate's work unit.
If you are a licensed pharmacist, you must fill in the licensed pharmacist.
Admission ticket number Please fill in the admission ticket number of the last exam.

The opinion of your unit is stamped with the official personnel seal of your current work unit.
Opinions of the superior competent department If the candidate's work unit has a superior competent department, the official seal of the superior competent department should be stamped in this column; If not, you don't need to fill in; If the candidate's unit is a state organ or institution, it must be stamped with the official seal of the higher authorities.
The opinions of the municipal examination management institutions can be left blank.
The examination and approval opinions of the Provincial Personnel Department can be left blank.
There are three columns in the qualification certificate: certificate management number, certificate number and qualification acquisition time, which candidates can fill in when they receive the qualification certificate.
