In order to prevent the failure of hospital information system from affecting the normal medical order of the hospital and ensure that patients can get timely and effective treatment under special circumstances, this plan is formulated in combination with the actual situation of our hospital, and all departments are expected to follow it in an emergency.
(A) Hospital information system failure reporting procedures
When each workstation finds that the computer can't enter the corresponding program, can't save data, can't access the network, and the application program doesn't work continuously, it should immediately report to the information network center. The staff of the center must attach great importance to the problems raised by each workstation, make records, feedback the fault information to each workstation in time after verification, and convene relevant personnel for discussion in time. If the cause of the failure is clear and can be recovered immediately, they should return to work as soon as possible; If the cause of the failure is unknown and the situation is serious and cannot be ruled out in a short time, it should be reported to the hospital leaders immediately, who will coordinate the work of all departments in the hospital under the condition that the network cannot run, so as to ensure the normal medical work in the hospital.
(B) Hospital information system fault classification
According to the cause and nature of the fault, it can be divided into three categories:
A kind of fault: server failure, backbone fiber damage, server database problems, unstable server work, network impassability and other software and hardware failures caused by downtime and network paralysis.
The second type of fault: local regional fault caused by switch, network fault, local problem of server database, unexpected data error, violation of workflow by some departments, etc.
Three types of failures: hardware, network, virus and single point failure caused by improper operation of each terminal computer.
According to the above fault classification levels, the treatment principles are as follows:
A kind of fault-led by the person in charge of the information network information department, organized and coordinated by the hospital, and led and coordinated by the information system fault recovery leading command group.
Class II failures-reported by the network administrator to the person in charge of the information department, and solved by the information center.
Three types of faults-solved by the network administrator alone, and registered in detail for maintenance.
(3) Main work when all kinds of faults occur
1, when a fault occurs.
(1) When the Information Department determines that it is a holistic fault, it should first report to the hospital leaders immediately, and organize the recovery work at the same time, and fully consider the impact of special circumstances such as holidays, large patient flow, personnel going out, and major activities in the hospital on the recovery time of the fault.
(2) When the overall network fault is found, each department will transfer the work to the corresponding emergency operation mode according to the degree of fault recovery time. The specific time limit is as follows:
(1) It can't be recovered within 30 minutes-the outpatient charge will enter the stand-alone operation mode.
(2) Unable to recover within 3 hours-hospitalization expenses, pharmacy, nurse workstation, doctor workstation, warehouse, physical examination, etc. Switch to manual operation.
2. In case of Class II failure.
(1) When a regional fault occurs, the Information Department shall organize relevant professionals to quickly find out the cause of the problem and notify the heads of relevant departments to formulate corresponding emergency plans.
(2) If the fault cannot be recovered in a short time, the following treatment methods should be taken according to different reasons:
(1) notify the software company to find and solve regional faults caused by software or database in time. Outpatient charges immediately start the stand-alone program, and other departments without stand-alone programs are turned into manual operations.
(2) Due to the regional failure caused by network or hardware problems, it is impossible to replace network equipment and related hardware. The department personnel involved in the failure can borrow computers from normal departments and notify the heads of relevant departments to coordinate their work.
(3) Due to the regional fault caused by virus, the connection between the fault area and the intranet should be physically cut off immediately, and the corresponding anti-virus treatment should be done.
3. When there are three kinds of faults.
(1) The Information Section will assign a special person to handle such failures. If it can't be solved in a short time, you can borrow a computer to deal with emergency work temporarily. Or manually, re-enter the computer after recovery.
Second, the computer network system backup and recovery scheme
In order to ensure the long-term stable operation of the hospital information system, the information network center should do a good job of backing up servers, networks, software and databases. In order to timely and effectively deal with system failures and minimize the impact on hospital business.
(a) server backup and recovery
The personnel on duty in the information department check the working status of the server every day and pay attention to whether there is an error message prompt. Conduct memory monitoring and disk space check.
(b) database backup and recovery
1, automatically backup 1 database to the local disk of the server in the background every day, and copy the backup files to the remote backup computer (backup computer with IP address of192.438+068.0.151) every month.
2. Check the database status, database space and running log every day, and notify the software company to find the reason immediately if any abnormality is found.
3. Copy the database backup files to the mobile hard disk for storage at the middle and end of each month.
4. It is suggested to improve the configuration of dual-machine hot standby system in the near future to provide stronger guarantee for system data security.
(3) Network backup and recovery
1. When the network equipment fails, solve it in time by replacing the corresponding equipment, and contact the equipment supplier to deal with the problem equipment. (At present, our hospital adopts the network topology structure of Vlan core switch, so it is suggested to purchase 1 Vlan core switch and convergence switch with the same model and configuration to ensure that the damaged network equipment can be replaced in time and the network operation can be resumed. )
2. When the backbone network is disconnected, the standby network cable can be temporarily laid, warning signs should be made to prevent damage, and personnel should be organized to repair it in time.
(4) Prevention and recovery of virus failure
1, check the upgrade and distribution of antivirus software every day to see if it works normally. (It is recommended that hospitals buy genuine online antivirus software)
2, check the regular antivirus, have found the virus, whether to deal with the virus, and understand the situation of the virus.
(5) Backup and recovery of software problems
1. Before updating the application software every time, make a good backup of the currently used software. Keep the software updated every time you back up.
2. Regularly check the automatic update of the software. If the workstation is manually updated, it will not be automatically updated, and find out the reasons.
3. If there is a large area after updating the software or it will affect the business, replace it with the normal software used before to ensure the uninterrupted operation of the business.
4, check the operation of medical insurance, medical interface every day.
5. Contact the developer to find the reason and deal with it in time when finding the software problem.
(6) Power supply system and computer room environment
1. Ensure the power supply of servers and network equipment, and adopt UPS and mains power supply.
2. Ask professionals to test and evaluate the storage performance of UPS regularly (at the end of each quarter).
2, waterproof, fire prevention (equipped with fire control facilities), regularly check whether all kinds of equipment and lines are damaged, if there is any damage, replace them in time.
Three, medical insurance network failure emergency plan
Due to the failure of medical insurance network in employee medical insurance center or resident medical insurance center, outpatient credit card consumption, special disease reimbursement and patient settlement failed. In order to ensure the orderly development of hospital medical insurance, make an emergency plan for network failure;
(a) when the medical insurance or financial settlement personnel find that the medical insurance network is out of order, which leads to abnormal consumption and settlement of the insured patients, they should give the insured patients a patient, meticulous and reasonable explanation.
