China Bank Shanxi Financial Social Security Card Cancellation and Loss Reporting

If the cardholder finds that his social security card has been lost but has not gone through the replacement procedures, he can go through the cancellation procedures (including formal cancellation of loss reporting) at the comprehensive service window of social security card and the China Bank loss reporting outlet window, cancel the social security account and bank account loss reporting of the card respectively, and resume the use of the card. If an agent is entrusted, the original valid identity certificate of the agent shall also be provided.

The specific procedures are as follows:

(1) The handling personnel or agent can go through the cancellation and loss reporting procedures at the social security card comprehensive service window and the social security card business window of the cooperative bank with the original and photocopy of their valid ID cards and the original and photocopy of their agent's ID cards;

(2) The agent or agent fills in the "Shanxi Social Security Card Business Processing Form" in the social security card comprehensive service window;

(3) After the staff of the social security card comprehensive service window and the social security card business window of the cooperative bank confirm the information, they shall go through the formalities of reporting the loss;

(4) The card can be resumed after the loss is reported.
