(two) the design, construction, safety evaluation and safety supervision units held by the "blasting operation unit license" and its copy.
(three) the blasting operation contract signed by the design and construction unit and the entrusting unit.
(four) the safety evaluation contract signed by the safety evaluation unit and the entrusting unit.
(five) the safety supervision contract signed by the safety supervision unit and the entrusting unit.
(six) the safety assessment report of the blasting design and construction scheme issued by the safety assessment unit.
(seven) submit the design scheme of blasting operation.
(eight) to submit the power of attorney of the legal person, the identity certificate of the person in charge and the identity certificate of the legal representative.
(9) Submit the relevant qualification certificates of the personnel participating in this blasting operation.
