5. 1. 1 During operation, in order to avoid the accumulation of hazardous energy in equipment, facilities or system areas or the accidental release of materials, all hazardous energy and material isolation facilities should be locked and labeled.
5. 1.2 Before operation, everyone who does not participate in the operation must confirm that the isolation is in place, lock and tag, and communicate with irrelevant personnel in time. The whole operation period, including shift change, should be locked and labeled.
5. 1.3 Locking and tag-out shall be carried out by the operator and the operator himself, and the safety lock and tag-out shall be placed in the correct position. Under special circumstances, if I have difficulty in locking, I want someone else to lock it under my nose. The safety lock key must be kept by the operator himself.
5. 1.4 In order to ensure safe operation, the operator may need additional isolation, locking and marking. When the operator has doubts about the effectiveness of isolation and locking, he can ask to test all isolation points again.
5. 1.5 When using the safety lock, a warning sign "Danger No Operation" should be affixed to the lock, and the lock must be listed. Under special circumstances, if the valve or power switch with special size can't be locked, warning signs can only be hung and locked after confirmation and written approval, but other auxiliary means should be adopted to achieve the same requirement of not locking.
5. 1.6 After locking and listing, it shall be confirmed that hazardous energy and materials have been removed or isolated through testing, otherwise all sources of hazardous energy and materials shall be regarded as not eliminated. After power failure, all electrical hazards should be checked by electrifying or discharging grounding.
5. 1.7 Identification of isolation points, isolation and formulation of isolation scheme shall be confirmed by local personnel, operators or both parties.
