To apply for the first, second and third-level qualifications approved by the province, a set of "Attachment Materials for Qualification Application of Construction Enterprises" (paper) is required. However, if the representative engineering drawings are involved (unable to be uploaded), it is necessary to fill in the Audit Opinion Form of the Municipal and County Authorities on the Representative Engineering Drawings of the Declaring Enterprise (printed) before the preliminary qualification examination, so as to prepare for the audit and signature, and upload it after confirmation and seal.
For the first-class qualification approved by the application department, two sets of "Attachment Materials for Qualification Application of Construction Enterprises" (paper) shall be produced. It is recommended to use standard A4 paper, catalogue, soft cover and back cover.
All the materials required for the attachments are photocopies, which shall be bound in the following order:
(1) Comprehensive Data (Volume I)
1, a copy of the business license of the enterprise as a legal person;
2. Original and photocopy of enterprise qualification certificate;
3. Articles of association;
4, the enterprise nearly three years of construction statistics;
5. The audited financial statements of the enterprise in the last three years;
6. The certificate and identity certificate of the legal representative of the enterprise;
7. Identity certificates, professional title certificates, employment documents, social security certificates (copies) of enterprise managers and technical and financial directors, and engineering performance certification materials required by technical directors according to relevant qualification standards;
8. If there are requirements for equipment and workshops, relevant certificates such as equipment purchase invoice or lease contract, workshop house property certificate or house lease contract shall be provided.
9, enterprise safety production license (labor subcontracting enterprises, concrete precast component enterprises, ready-mixed commercial concrete and other enterprises can not provide);
Among them, the enterprise applying for qualification for the first time does not need to provide the materials mentioned in items 2, 4, 5 and 9 above, but should provide the documents of enterprise safety production management system.
Enterprises applying for qualification for the first time shall apply from the lowest qualification, and new enterprises across the year shall provide the audit report of the previous year.
10, to apply for special qualification, in addition to the above materials, you should also provide:
(1) bank credit certificate of the enterprise in recent three years;
(two) the enterprise has handed in the construction business tax receipt or overseas project settlement certificate in the past three years;
(three) the certificate or valid approval document of the enterprise technology (R&D) center or sub-center at or above the provincial level;
(4) National construction method appraisal documents and patent technology appraisal certificates;
(5) copy of the notice (or order) of the national science and technology progress award certificate or the national and industrial standards for engineering construction edited by the editor, cover, catalogue, preface and introduction.
1 1. Other materials required by relevant qualification standards.
(2) personnel data (Volume II):
1. The identity certificate, registration certificate and endowment insurance certificate (roster, payment invoice) of the registered personnel listed in the Application Form for Qualification of Construction Enterprises, and the social security personnel roster shall be filled in according to the Social Insurance Payment Form of the Ministry of Construction.
2. The professional title certificate, identity certificate and social security certificate (roster and payment invoice) of non-registered professional and technical personnel required by the qualification standards of construction enterprises, and the social security personnel roster shall be filled in according to the social insurance payment form of the Ministry of Construction.
3. The newly established enterprise shall provide the pre-registration certificate of the builder according to the qualification standard and list it in the application form. After the qualification is approved online, it will be officially injected into the CCB business department, and the ministries and commissions can issue certificates.
4. Some qualification standards require enterprises to have professional title certificates, identity certificates and endowment insurance certificates of special professional and technical personnel, and also provide corresponding certificates and professional certification materials. The roster of social security personnel shall be filled in according to the Social Insurance Payment Form of the Ministry of Construction.
5. Personnel with intermediate or above titles should apply for endowment insurance. Construction engineers or small project managers and technical and economic personnel are independent of each other. Enterprise personnel registered or employed in more than two enterprises shall not be recognized, and the unit on the certificate must be consistent with the name of the reporting unit.
(3) Engineering performance data (Volume III):
1, engineering contract, bid-winning notice;
2, in line with the provisions of the state completion acceptance list (record) or quality inspection data;
3, the above information can not reflect the technical indicators, but also provide engineering photos, drawings, project final accounts, etc. Reflect the requirements of technical indicators.
(Note: External employees must sign a 2-5-year employment contract, which is bound by engineering and then by economy, including ID card and letter of appointment).
(4) Where an enterprise applies for restructuring, division or merger and needs to re-approve its qualification, it shall provide the following materials in addition to the above materials (included in the first volume of comprehensive materials):
1, enterprise reorganization, division and merger plan (including the division of assets, personnel and project performance between the new enterprise and the original enterprise);
2, enterprise restructuring, division, merger approval documents or resolutions of the shareholders' meeting or workers' congress;
3, enterprise restructuring, division should provide the financial statements and statistical statements of the enterprise in the first three years of restructuring and division;
4. For business combination, provide the financial statements, statistical statements and the latest consolidated statements of the last three years before the business combination;
5. A capital verification report issued by an accounting firm.
Note: The paper materials for applying for provincial examination and approval qualification are bound into one volume.
Three, the application department to generate the "enterprise qualification application acceptance information form".
When the application department approves the first-level qualification information, it shall report the information on the acceptance of qualification application through the website of the Ministry of Construction. The process is as follows: the website of the Ministry of Construction → the service hall → the information reporting software for enterprise qualification application acceptance → the information reporting for enterprise qualification application acceptance, and the information form is printed and stamped with the official seal of the enterprise.
