2. Unsafe state of things:

3. Defects in management:
(1) Some managers don't know enough about the importance of safety work ideologically and regard it as dispensable. They treat safety work with numbness and negative behavior every day, and their awareness of safety legal responsibility is extremely weak.
(2) Safety rules and regulations (including equipment inspection), operating rules, post responsibility system, corresponding preventive measures, safety preventive measures and logistics management procedures are not established, sound or imperfect.
(3) Some managers do not study, understand, implement and thoroughly implement the company's safety rules and regulations, do not implement the "three simultaneities" of engineering construction and "five simultaneities" of daily management, and only pay attention to production indicators, ignoring safety inspection, education and hidden dangers rectification.
(4) A few management personnel failed to diligently, responsibly and thoroughly ensure the management measures and effective preventive measures for the health and property safety of personnel in the jurisdiction area in accordance with the company's safety management system and safety management requirements, combined with the production characteristics and working environment of the jurisdiction area, and the safety management was poorly implemented.
(5) Some managers have defects in safety knowledge, safety management ability and means.
(6) Effective reward and punishment measures to ensure the implementation of rules and regulations need to be improved and strengthened.
(7) Managers do not pay attention to "management requirements" such as safety announcements and notices.
