According to the provisions of Article 23 of the Regulations on Work-related Injury Insurance, the labor ability appraisal needs to apply to the municipal labor ability appraisal committee where the work-related injury employee unit is located, and provide relevant information on the work-related injury determination decision and the medical treatment of employees.
3. According to the provisions of Article 8 of the Measures for the Administration of Labor Ability Appraisal of Workers with Work-related Injury, an application form for labor ability appraisal shall be filled in and the following materials shall be submitted:
(a) the original and a copy of the "work-related injury determination decision";
(2) Complete medical records such as valid diagnosis certificates, copies or duplicates of inspection reports that meet the relevant provisions of medical institution medical record management;
(three) the original and photocopy of the valid identity certificate such as the resident ID card or social security card of the injured employee;
(four) other materials as prescribed by the labor ability appraisal committee.
